How to Start Your Own Practice as a Nurse Practitioner

You may be thinking, “What do I need to do to open my own business as a nurse or nurse practitioner (NP)? How hard is it going to be? What support will I need?”

Opening your own business is not quite like nursing school or your first nursing job. It’s similar in that you will:

  • Still need to put in a lot of work
  • (Potentially) pull all-nighters
  • Need a great support network

But like nursing school and that new job: In the end, it’s worth it.

We spoke with business owners Wendy Jules, RN, BSN, and Peggy Roberts, DNP, WHNP-BC, who know a thing or two about nurse entrepreneurship. Both opened their businesses during the COVID-19 pandemic!

8 Pandemic-Proof Steps to Opening Your Healthcare Business

As registered nurses (RNs), Jules and her cofounder Carla Nelson knew they had the skill set to open a healthcare business. Their education and nursing experiences gave them confidence.

Being passionate about the services or products you are offering is key, they say. If your passion doesn’t shine through, your clients won’t be passionate either.

Roberts also notes the importance of taking your time. Due to the pandemic, it took Roberts almost a year to open the doors to Trust Women’s Healthcare. Although this was a long time, starting a business is a huge responsibility. There are many risks involved, and you want to do it right.

“This isn’t a race; it’s a marathon,” Roberts says.

Ready to take the next step? Here are eight essential steps you need to take when opening up a new business.

1. Know Your ‘Why’

Although opening a business can be profitable, knowing your ‘why’ is even more important than making money. If you want to have a successful, scalable business, take some time and write down exactly why you want to be a business owner.

For both our nurse entrepreneurs, the ‘why’ behind their businesses hit home in personal ways, including representation, accessibility, and giving back to their communities.

Jules and Nelson noticed a major lack of representation of Black skin care providers in their community. This was when they came up with the idea of opening Fleur De Lis Beauty & Esthetics, a medical spa in Brooklyn, New York.

“Our goal was to provide easy access to corrective skin care,” Jules says. “We also wanted to create a beautiful space where clients can come, relax, and make self-care a priority.”

Roberts penned Trust Women’s Healthcare as a way to give back to her community. After completing her doctoral studies, she was astounded to learn the overwhelming healthcare disparities Black women face in the United States. Black women are 3.5 times more likely to die from complications from pregnancy than white women, prompting a need for advocacy for patients of color .

After seeking other leadership roles and talking to colleagues, Roberts decided opening a women’s health clinic was the best way to give back to her community. She also hopes to decrease women’s health disparities among women of color.

“I always believed if you are not invited to the table, you create your own,” Roberts says.

2. Have Your Paperwork in Order

You need paperwork to get your business off the ground. According to Jules, this includes:

  • Choosing a name for your practice
  • Figuring out how to structure your business (e.g., limited liability company, sole proprietorship, partnership)
  • Getting an Employer Identification Number

There are many types of business structures you can choose from. First, you should decide which type of business will work best for you and your company.

Three Common Types of Business Structures

  • Limited liability company : An LLC lets you take advantage of both a partnership and a corporate business structure. You must file your LLC with the state where you practice.
  • Sole proprietorship : A sole proprietorship is when one person owns and runs the daily operations of the business.
  • Partnership : A partnership is when two or more people own and run the business’s daily operations.

These business structures have pros and cons, so remember to research to find the best fit for your goals.

3. Know How You’ll Fund Your Business

You want to open your nursing business, but how will you fund it? There are many ways to fund your business. You can use the money you’ve saved up, or you can borrow from a bank.

Since Jules and Nelson opened their business during the pandemic, they encountered hurdles while securing funds. Many banks were not willing to support their business.

“We initially relied on our family, friends, and ourselves, emptying out our 401ks to fund the business,” Jules says.

Luckily, they partnered with Carver Bank, the largest African American managed bank in the United States, who helped them navigate the banking system through the pandemic.

To secure funds, it’s important to gather enough information to make informed decisions. This includes:

  • Speaking to family and friends
  • Calling several banks to find out which ones will work with you
  • Consulting a certified public accountant (CPA)

Speaking with a CPA can save you a lot of time and prevent mistakes. The benefits of consulting with an accountant can help set up your business successfully, Roberts says.

4. Speak to a Healthcare Attorney

When opening a business, you may think all attorneys are created equal. They aren’t. A healthcare business is specialized. Roberts suggests seeking out attorneys who are fully versed in healthcare law.

“Really do your research and interview your attorney,” Roberts advises. This will help with any unforeseeable issues.

Also, make sure to budget for an attorney. They can be expensive. Roberts is based in New York, so her attorney came with a hefty price tag.

Make sure to do your research before meeting an attorney. Have your questions ready. The more prepared you are, the lower the price tag.

5. Obtain Certifications Your Practice Requires

If you want to open a medical spa, getting certifications will allow you to provide various services and attract more clients. Jules suggests getting the certifications your practice is going to require.

“For myself, we decided to do intravenous (IV) hydration, so I undertook certifications in IVs and injectables,” Jules says.

Build as much outside knowledge as you can to bring value to your clients and wealth into your practice.

6. Understand Your Business Partner’s Role

Opening up a business can be confusing and emotional if you have a partner. Personalities can clash. That can be draining and take away from your business goals.

Jules and Nelson are sisters, roommates, and business partners. “We saw each other every day,” Jules says.

To combat possible roadblocks, Jules suggests understanding your partner’s business language.

For Jules and her sister-turned-business partner, this meant separating their personal relationship from their business relationship. By doing this, there is less room for role confusion.

“If everyone is doing everything, it’s difficult to keep track of your success or your failures,” Jules says.

7. Research and Find Resources

You can’t start a business without doing research. You should use your resources for advice.

If you are an NP, research the practice authority in your state first.

Note that an NP’s scope of practice varies from state to state. Some states have full authority; some have restrictive authority. Be sure to know which level your state offers.

8. Network, Network, Network

In the age of social media, finding resources to help guide your path of nurse entrepreneurship shouldn’t be too difficult. Roberts suggests leveraging social media platforms like Instagram or LinkedIn to network and access resources available to you. You can get a lot of information from nurses and NPs who are opening similar businesses, too.

With endless access to advice on social media, it’s important to separate reputable sources from misleading ones.

If you are suspicious, Roberts suggests checking out the website or LinkedIn profile for the businesses or business owner you’re looking to take advice from. Follow them for awhile and see who else they follow. Finally, make a decision based on your findings.

Remember that you have many transferable skills as a nurse that can lend to successful entrepreneurial adventures. It takes making the first step to get to where you want to be.

  • Attending networking events for nurses and business owners
  • Taking advantage of professional nursing organizations by becoming a member of a nursing or an NP association

Use this technology to fill-in a sparce schedule, meet with new types of clients, or even work with other businesses such as health and wellness brands or media companies as a consultant or resident expert.

NPs are advanced practice nursing professionals who provide many of the same services as physicians, but often with a more holistic approach. With the Health Resources and Services Administration (HRSA) reporting in 2023 that 259 million Americans live in primary and mental health care workforce shortage areas, many states are exploring ways to allow NPs more autonomy to provide services once offered only by doctors.

Many states already permit practice autonomy for NPs, and others may follow — making it a great time for nursing professionals to consider pursuing a career as an NP and starting a private practice. However, what’s the path to starting a nurse practitioner private practice?

A Doctor of Nursing Practice (DNP) degree provides the highest level of nursing knowledge and leadership skills, which can be valuable preparation for starting an NP private practice.

What Is a Nurse Practitioner Private Practice?

In 2020, most NPs worked for others in locations such as hospitals and group practices, according to the American Association of Nurse Practitioners (AANP). But a growing number were choosing to work in private practice

Private practice, AANP indicates, was among the top 10 work settings for NPs in 2020.

Nurse practitioner private practices are independent businesses that NPs operate themselves to provide patient care. About 4% of NPs reported to AANP that they provided services in these settings.

Nurse Practitioner Services

Whether they’re working for other health care professionals or in their own practice, NPs offer services that blend the treatment of medical conditions with a focus on disease prevention and health education.

NPs are a type of advanced practice registered nurse who perform work that often is similar to that of a physician. NPs’ scope of responsibility varies by state, but the types of care that NPs typically offer include the following:

  • Managing patients’ overall health care
  • Diagnosing and treating acute conditions, such as infections and injuries, and chronic disorders, such as diabetes and high blood pressure
  • Prescribing medication, including controlled substances
  • Ordering, conducting, and interpreting medical tests and X-rays
  • Counseling patients about lifestyle choices that promote wellness

Nurse Practitioner Specializations

NPs provide primary, acute, and specialty care, but the overwhelming majority of these professionals hold a certification in some type of primary care. In 2022, AANP reports, 88% of NPs held a certification to provide primary care, offering long-term assistance for various health needs — from preventive procedures to disease management.

Each NP chooses a particular patient population or condition as a focus of care. Family nurse practitioner (FNP) is the most common area of specialization, according to AANP. In 2022, about 70% of NPs specialized in providing primary care for patients from infancy to older adulthood.

Among the other common areas of specialization for NPs are:

  • Adult-gerontology primary care: Offering primary care to patients from young adulthood to older adulthood
  • Adult-gerontology acute care: Offering short-term care for acute conditions to patients from young adulthood to older adulthood
  • Psychiatric mental health: Focusing on treatments that support patients’ mental health
  • Women’s health: Providing various medical services to women, including OB-GYN care
  • Pediatric: Caring for patients from infancy to young adulthood

Why Start a Nurse Practitioner Private Practice?

NPs who are considering starting a private practice might wonder what the advantages are. The reasons that NPs go into business for themselves range from following their own interests to meeting community needs. Often NPs who choose to start a private practice do so for the independence it can give them to choose their own hours and services.

Some of the primary benefits of opening a nurse practitioner private practice include the following:

Gaining Independence

Leading a private practice can give NPs more independence than they might have when working for others. For example, they can set their own:

  • Work schedule
  • Operating procedures

Closing Health Care Gaps

The Association of American Medical Colleges (AAMC) projected in 2021 that there could be a shortage of as many as 124,000 primary and specialty care physicians by 2034. To compensate for this deficit and meet the growing demand for health care, the organization suggested expanding the role of other advanced health care professionals, including NPs.

NPs with their own practice can match their specialties and areas of expertise to a community’s medical needs. HRSA, for example, noted that the U.S. had a shortage of more than 17,000 primary care practitioners as of February 2023. The nation also needs roughly 8,000 more mental health practitioners to accommodate the needs of those living in shortage areas. These care gaps are especially prevalent in rural areas.

Increasing Income Potential

Operating their own practice gives NPs greater authority to establish their own rates, thus dictating the earnings they will receive. Unlike most clinics and hospitals, private practices can establish their salaries without regard to the pay scale.

It should come as no surprise to anyone reading NP Business, that the most common question we receive is “ How do I start a practice? ” As we get ready to close out 2023, I wanted to post a playlist of our most popular podcasts and blog posts about starting a practice. I’ll follow this up with a post about growing and operating a practice.

This one podcast/video episode is important, and you could start and end with this episode alone. There is so much that must be done when starting a practice, all at once, and this framework (which is how I teach this) will help keep you sane and organized.

The initial part of starting a practice involves research and planning. Of course, a big part is working with numbers s and building assumptions to determine if your plans will be feasible . Understanding what it costs to start and run a practice is crucial.

There are many moving parts to starting a business, and sometimes, because it’s often “just you” seeing patients, clinicians (not just NPs) will opt for the easiest structure. But that could be a huge mistake. Listen in as I tell you why there are TWO legal structures you want to stay away from.

This episode covers why you need an NPI-2 , particularly if you’re just getting started, and those already in practice, but may not have set things up properly. It does not matter if you are billing insurance, or if you are a cash practice. This is essential information !

An estimated 100 million people in the United States—almost a third of the population—don’t have a primary care provider. Nurse practitioners could bridge this urgent health care gap, but social stereotypes that portray nurses as subordinate to doctors, along with archaic state regulations, prevent these qualified professionals from doing so.

The current care gap is driven mostly by a lack of doctors. Medical students are choosing higher-paying specialties over practicing primary care. Often, this is to help them pay their medical school debt.

The nursing community is poised to fill that gap. There are 5.2 million registered nurses in the U.S. , and they are increasingly earning degrees and certifications that qualify them as primary care providers. More than 70 percent of RNs hold a bachelor of science degree, a number that is growing. Then there are 385,000 licensed nurse practitioners—a designation that allows them to order tests, diagnose patients, and prescribe treatments, just as a doctor would. Nearly 90 percent of nurse practitioners working in the U.S. today have a master’s or doctorate degree and are fully certified to practice in areas such as pediatrics, family medicine, women’s health, adult medicine, or gerontology.

It usually takes at least seven years and an average of 7,840 hours of clinical training and work experience to become a nurse practitioner. Nurse practitioners are well-trained for primary care work—and evidence shows they deliver it effectively, and often at a lower cost than doctors. Studies show that patients of nurse practitioners received the same quality care from nurse practitioners as from doctors, and that they feel highly satisfied with their care.

Other research shows that being seen by a nurse practitioner incurs costs similar to or less than being treated by a physician. One study , which analyzed two years of data from more than 47,000 medically complex patients with diabetes at 566 Veterans Administration facilities, found savings of at least hundreds of dollars per patient: $1,300 for hospitalization or emergency visits, $500 for outpatient care, and $300 in pharmacy costs. Another study , which analyzed four years of data from 475,000 Medicare beneficiaries, found that the patients of nurse practitioners received high-quality care while costing less for office visits as well as emergency room visits, hospital stays, nursing homes, and specialty care.

In 27 states , the District of Columbia, and U.S. territories, nurse practitioners are fully authorized to practice as independent clinicians. They conduct physical exams, make diagnoses, order and interpret lab tests and imaging studies, prescribe medications, provide treatment, and perform procedures, just like primary care physicians do. But when people think about their primary care, they think of the word “doctor.”

If policymakers and other industry decision-makers can help patients in the U.S. see nurse practitioners as primary care providers, patient access and health outcomes would improve, and health care costs would go down. Updating the laws and regulations in the states that don’t yet authorize nurse practitioners to work this way would help. But real change needs public health and health care practitioners to collaborate to change people’s minds. Patients and the public need to learn they can turn to nurse practitioners for expert, quality care.

To be sure, nurse practitioners are not physicians. But access to primary care nurse practitioners trumps no access every time. There is plenty of need—and space within health care—for physicians and nurse practitioners to work independently and collaboratively for the good of all the people in the United States who need care.

Out of the Centre

Savvino-storozhevsky monastery and museum.

Savvino-Storozhevsky Monastery and Museum

Zvenigorod's most famous sight is the Savvino-Storozhevsky Monastery, which was founded in 1398 by the monk Savva from the Troitse-Sergieva Lavra, at the invitation and with the support of Prince Yury Dmitrievich of Zvenigorod. Savva was later canonised as St Sabbas (Savva) of Storozhev. The monastery late flourished under the reign of Tsar Alexis, who chose the monastery as his family church and often went on pilgrimage there and made lots of donations to it. Most of the monastery’s buildings date from this time. The monastery is heavily fortified with thick walls and six towers, the most impressive of which is the Krasny Tower which also serves as the eastern entrance. The monastery was closed in 1918 and only reopened in 1995. In 1998 Patriarch Alexius II took part in a service to return the relics of St Sabbas to the monastery. Today the monastery has the status of a stauropegic monastery, which is second in status to a lavra. In addition to being a working monastery, it also holds the Zvenigorod Historical, Architectural and Art Museum.

Belfry and Neighbouring Churches

Located near the main entrance is the monastery's belfry which is perhaps the calling card of the monastery due to its uniqueness. It was built in the 1650s and the St Sergius of Radonezh’s Church was opened on the middle tier in the mid-17th century, although it was originally dedicated to the Trinity. The belfry's 35-tonne Great Bladgovestny Bell fell in 1941 and was only restored and returned in 2003. Attached to the belfry is a large refectory and the Transfiguration Church, both of which were built on the orders of Tsar Alexis in the 1650s.

To the left of the belfry is another, smaller, refectory which is attached to the Trinity Gate-Church, which was also constructed in the 1650s on the orders of Tsar Alexis who made it his own family church. The church is elaborately decorated with colourful trims and underneath the archway is a beautiful 19th century fresco.

Nativity of Virgin Mary Cathedral

The Nativity of Virgin Mary Cathedral is the oldest building in the monastery and among the oldest buildings in the Moscow Region. It was built between 1404 and 1405 during the lifetime of St Sabbas and using the funds of Prince Yury of Zvenigorod. The white-stone cathedral is a standard four-pillar design with a single golden dome. After the death of St Sabbas he was interred in the cathedral and a new altar dedicated to him was added.

Under the reign of Tsar Alexis the cathedral was decorated with frescoes by Stepan Ryazanets, some of which remain today. Tsar Alexis also presented the cathedral with a five-tier iconostasis, the top row of icons have been preserved.

Tsaritsa's Chambers

The Nativity of Virgin Mary Cathedral is located between the Tsaritsa's Chambers of the left and the Palace of Tsar Alexis on the right. The Tsaritsa's Chambers were built in the mid-17th century for the wife of Tsar Alexey - Tsaritsa Maria Ilinichna Miloskavskaya. The design of the building is influenced by the ancient Russian architectural style. Is prettier than the Tsar's chambers opposite, being red in colour with elaborately decorated window frames and entrance.

At present the Tsaritsa's Chambers houses the Zvenigorod Historical, Architectural and Art Museum. Among its displays is an accurate recreation of the interior of a noble lady's chambers including furniture, decorations and a decorated tiled oven, and an exhibition on the history of Zvenigorod and the monastery.

Palace of Tsar Alexis

The Palace of Tsar Alexis was built in the 1650s and is now one of the best surviving examples of non-religious architecture of that era. It was built especially for Tsar Alexis who often visited the monastery on religious pilgrimages. Its most striking feature is its pretty row of nine chimney spouts which resemble towers.
