Basically questionnaire is a structured interview which subject or respondant is asked a series of questions according to a prepared and fixed interviewing schedule - the questionnaire (Brace, 2004). In order to test a hypotheses or answer a research question, survey researcher must measure the concepts they reference as precisely as possible, striving for the least amount of error. To test the hypotheses, researcher must use variables to represent specific concepts. This involves paying careful attention to the design of each questionnaire item and response option. This is because to ensure the instrument is valid and reliable to measure what the researcher want the item or instrument to measure. Questionnaire design is a critical component of many research projects. A good, valid and reliable administered questionnaire will be able to answer the research question while poorly designed questionnaire will not only waste time and energy and also provide useless data (Frazer & Lawley, 2000) According to Ruel, Wagner and Gillespie (2016) the main goal of a questionnaire of a survey research is to gather data in an individual’s demographic, behaviours, personal details, attitudes, beliefs and opinions.
