Enter your Name and Birth Details. The name should match the one printed on your i-485 receipt.

If your name is different on your passport or has spelling mistakes, then we strongly suggest creating a one and the same person affidavit and sending it with your package.

Enter your A-number . This is required for USCIS to locate your i-485 file in their mail/storage room.

Interfiling Risks vs Benefits

Please note that there is no official medical interfiling process designed or published by USCIS.

The interfiling simply means that you send the documents as per your wish and then request USCIS to add them to your file. All this is done without USCIS requesting these documents through the official process of RFE (request for evidence).

Since it is an unsolicited request, USCIS may or may not add these documents to your file.

Many people have seen success with sending the i485 medicals without RFE but there is no guarantee.

Also, your attorney may or may not agree to send it without RFE. So, it is entirely your choice. You can send the medical interfile letter directly yourself without asking your attorney to do it.

Benefits of interfiling

There is a 50% chance that your interfiled medical application will end up in your green card file at the USCIS service center.

Now, a 50% chance is a very good chance given that there are thousands of applications waiting to receive medical RFE.

If your interfile request is successful, then your file has a better chance of

  • Getting ahead in the queue even though your PD date or the i485 receipt maybe after someone who has an earlier PD date.
  • The USCIS officer working on your case may not issue medical RFE at all if they see your interfiled medical in your file. This will easily save about 30 days of processing time on your case.

Risks of interfiling

There is virtually no risk in sending the i693 form without an RFE except that you may lose the doctor’s fee if USCIS does not add it to your file.

The Doctor usually charges about $350 per application and you stand to lose it. We know that most Indians will happily take this risk even if it gives them a slight chance to get ahead in line.

  • Your pending i485 application will not see any adverse effect simply because you tried to interfile.
  • USCIS will issue the medical RFE. In this case, you will have to redo the medical and send it with an RFE letter.

Our Suggestion

Send the sealed medical letter along with the cover letter and the i485 receipt bar code.

There is nothing to lose here. Just try every possible option to help move your application ahead in the queue and get a green card faster. You may have waited for a decade for your Green card. Do not leave any stone unturned to push your application forward.

We understand that this effort may go to waste but think of the situation where it may get accepted. Your application will certainly save time and may get approved earlier than others.

On Mar 31, 2023 – USCIS removed the 60-day rule of doctor signature . This means that i693 will be accepted even if the doctor signed it more than 60 days ago.

Earlier, the i693 form was considered valid only if the doctor had signed it within 60 days of submitting it to USCIS. There is no such requirement now after 31 Mar 2023.

You should ideally send the medical interfile package to the address printed on your i485 receipt notice. The service center address will be mentioned in the bottom left corner of the i-485 receipt notice.

Sending to USCIS Field Office?

You can also send it to the USCIS field office if your green card file has been transferred there. You should confirm this by first checking with USCIS Emma chat.

How to Get Your File’s Field Office Location ?

To ask for the current location of your Green card file from Emma’s agent, follow these steps:

  • Visit  uscis.gov and click the top right “Ask Emma” button to start chatting with Emma’s agent.
  • Once the chat box is open, type “ representative “.
  • When you see the list of options, click “ case status “.
  • Click “ connect to live chat ” and enter your i-485 receipt number.
  • When prompted for your request, type “ location of file “.
  • At this time, you should be connected to a live agent.
  • Ask them “ I would like to know where my I-485 file is right now “? The agent should provide the name of a service center or local field office.
  • Once you know the field office, ask them if they have the office’s address. If they give it, use it to send your medical interfile letter. If they can’t give the address, you can search it on the USCIS website by field office name or search on Google.

You should send the medical interfile letter to the USCIS service center address that’s printed on the left bottom corner of your i-485 application receipt notice. The receipt notice is usually called form i-797C (notice of action).

USCIS usually does not send any receipt or acknowledgment for interfiling letters. They send receipts when they have requested the documents by way of issuing RFEs.

Yes, you can use the same interfile letter as a cover letter for your medical RFE response. It certainly helps USCIS since you are adding your i485 receipt bar code.

I've included my three cover letters (General, for I-130 and for I-485).  I've explained that we'll be performing our medical exam at a later date (we need to travel pretty far to get that done and I don't know when I can get some time off of work and I'm ready to get this thing in the mail) but I didn't give any details. Is that a problem?   We have no lease since we're living with my folks at the time being, but I'm hoping this initial evidence will be sufficient.  We've been married for over five years now.  Also, I included our Venezuelan marriage certificate (copy) and the Colombian marriage certificate since our marriage was registered in Colombia just to be thorough.

How to Interfile Your Medical Exam Packet with Pending I-485

Our office receives many questions a day about whether an individual with a pending I-485 green card application should send their I-693 medical exam packet to USCIS before it is requested in a Request for Evidence (RFE). It is possible to send the medical packet to USCIS before they send an RFE requesting the medical exam through a process called interfiling. An individual with a pending I-485 can send the sealed medical exam packet in advance and request USCIS to interfile the medical record with the pending I-485 application. In many cases USCIS will accept the interfile, although it is not 100% guaranteed. It is still possible that USCIS will send an RFE requesting your medical records. Or, USCIS may request for you to bring your medical exam packet at the time of the in-person interview.  If this happens, you must get your medical exam done again and send a new medical exam packet to USCIS.

How do I send an interfile request to USCIS?

Prepare a medical interfile cover page to the USCIS Service Center address stated on your I-485 application receipt notice. Below is sample text you can use on the cover page to send to USCIS:

Will I get notice that USCIS has received my interfiling request?

No, USCIS generally does not send receipts or any type of notice that is has received your medical records.

I sent my medical records to USCIS via interfiling, but now I’ve received an RFE asking for the medical records. What do I do?

You should get your immigration medical exam re-done and respond to the RFE with the new, sealed medical packet.

For how long is my medical exam valid?

Once the civil surgeon signed Form I-693, it must be filed with USCIS within 60 days. The medical packet retains its evidentiary value for 2 years from the date the civil surgeon signed the I-693.

Interfiling Medical Exams for I-485 Filings

USCIS requires that certain individuals seeking immigration benefits, such as permanent residence, must “establish that he or she is not inadmissible on health-related grounds.” Form I-693 is used by I-485 applicants to show they “are free from any conditions that would render them inadmissible under the health-related grounds.”

USCIS states that a medical examination, or completed Form I-693, is valid for 60 days after the civil surgeon physician signs the form. Once submitted to USCIS within the 60-day period, the exams will remain valid for up to 2 years while USCIS adjudicates your Form I-485.

What happens if you didn’t submit your medical form with your I-485?

You have a few options for submitting your Form I-693 after your I-485 has already been filed. Most commonly, USCIS will request the medicals when your case is in the final stages of adjudication by issuing a Request for Evidence (RFE). The I-485 applicant can submit the medical exams to the address on the RFE. You can also bring your medical exams with you if you are scheduled for an in-person interview at a USCIS field office.

In recent months, we have seen some employment-based cases approved without an interview at the field office. If you see that USCIS is processing cases near your priority date and your case may be nearing adjudication, you can also choose to “interfile” or mail USCIS your medical exams to have them linked to your case ahead of an RFE.

What are the risks of interfiling my medical exams?

The main risk is that USCIS may not be able to locate your file and attach the medical exams in a timely manner. For example, if the file has been transferred to a field office and you send your medicals to the service center, there could be a delay in connecting the documentation with your case. If your medical exams are lost or not connected within 60 days, you may have to pay the fees to obtain a new set of medical exams and send to USCIS when requested in an RFE or when you are scheduled for an interview.

Tips for Interfiling Medicals

1. Confirm the actual location of your case with USCIS. 

After checking your case status on your USCIS portal , you can also chat with Emma on the USCIS website (an automated tool that could also connect you to an agent for a live chat) or speak with someone at the USCIS Contact Center to ensure the location of your case.

2. Check the mailing address for the location.

If you have not received an RFE requesting the medical exams, you need to check the acceptable mailing addresses for the appropriate service center or field office. The USCIS website has an office locator function, in addition to listing the direct filing addresses for Form I-485 .

3. Include your case details and prepare the package for filing. 

Make sure you have your I-485 receipt numbers and medical exams dated within the last 60 days. Include a copy of the G-28 if applicable. Download a sample medicals cover letter.

4. Ship the medicals with a trackable delivery service. 

Use a delivery service that provides tracking numbers so that you can confirm the date and time of arrival. Keep a record of the confirmation delivery with your files.

How to Prepare & Submit the Form I-693

How to fill out form i-693 (for the applicant).

The I-693 includes 10 parts. As the Applicant you only need to fill out Part 1 before your doctor appointment. You will also need to write your name and Alien Number (when you have one) at the top of each page of the form. This should be an 8 or 9 digit number starting with the letter A and can be found on any letters or notices you may have received from the Department of Homeland Security.

To fill out Part 1 you will need:

  • Your full name (your legal name)
  • Your address – the place you live, PO boxes will not work
  • Your date of birth
  • The city, town, or village in which you were born
  • The country you were born in
  • Your Alien Registration Number (A-Number) if you have one
  • Your USCIS online account number, if you have one

As the Applicant you will also need to fill out Part 2. This part is where you give some contact information and sign the form. You MUST fill out this section with the civil surgeon present to witness you filling it out.

Doctor Appointment

To find a USCIS certified surgeon:

  • Go to uscis.gov
  • Select “Find a Doctor”
  • Enter your ZIP Code
  • Click “Search”

A list of doctors in your area should appear. Select one and make an appointment by calling the number next to it.

How to fill out Form I-693 (For the Interpreter)

  • You will be required to fill out Part 3. This includes:
  • The name and address of the business or organization (when applicable)
  • Your daytime telephone number
  • Your mobile telephone number (if you have one)
  • Your email address (if you have one)
  • Your signature and the date

How to fill out Form I-693 (For the Civil Surgeon)

As a USCIS certified civil surgeon you are required to report true and accurate results from your examination.

To do this you will need to take down the information from the Applicant’s government-issued identification. You will also be responsible for witnessing that the Applicant fills out and signs Part 2.

You will need to follow the most recent Department of Health and Human Services regulations and the Centers for Disease Control and Prevention guidelines. Those guidelines can be found here .

You may also be asked to make referrals and file case reports. These processes can also be found in the CDC guidelines .

When filling out the I-693 you will need to:

  • Verify the Applicant’s Identification Information
  • Give a summary of the medical examination, after the exam and any follow-ups necessary are completed
  • Give your contact information and sign the form after the examination is complete, certifying that you are a legal civil surgeon. (If you are performing the assessment for a refugee adjustment applicant you will need to include the licensed physician that was present during the examination and the official stamp or raised seal of the health department. If you are a military physician performing the examination for a member or veteran of the U.S. Armed Forces or Coast Guard or certain eligible dependents, the licensed military physician’s signature – who is operating under the blanket civil surgeon – is required as well as the official stamp or raised seal of the military treatment facility.)
  • Fill out the worksheet included in the I-693
  • Include the results of any lab work or other studies that were required to determine whether the Applicant is inadmissible to the U.S. on health-related issue
  • If a referral is required, fill out Item 5 in the worksheet found in Part 7. The physician referred will then need to fill out Part 8. (If a referral is not necessary, leave Item 5 in the worksheet blank.)

Is there a filing fee?

There is no filing fee for the I-693.

How to file the I-693

  • Make an appointment with a civil surgeon certified by USCIS.
  • Fill out Part 1. In your I-693. (Do not sign it yet. You must sign the form in front of the civil surgeon.)
  • Keep your appointment with the civil surgeon. If more visits are necessary, attend all of those as well. Make sure to bring proof of your identity and any medical records you may have with you to your appointment. For proof of identity bring a valid government-issued photo identification (i.e. valid passport or driver’s license). If you are under 14 documents must show your name, place of birth, parents’ full names, and any other information that can identify you (i.e. birth certificates or affidavits, include translations when necessary).
  • The civil surgeon should complete the I-693, witness you signing it, and make a copy. The civil surgeon should seal the original I-693 in an envelope and give it to you to submit to USCIS and give you the copy they made.
  • You should then submit the sealed letter to USCIS. If you are filing the I-693 because you are applying for and adjustment of status you may do this in one of three ways:
  • Mail it with your Form I-485 to the address that matches your reason for filing the I-485. A list of reasons for filing the I-485 and the address needed can be found here .
  • Mail the I-693 after you’ve mailed the I-485 to the address specified in your latest communication with USCIS.
  • Submit the I-693 in person when you go in for an interview at a USCIS field office.

If you are not applying for an adjustment of status, follow the instructions given by the office requesting the medical examination.

Form I-693 Medical Exam

U.S. Citizenship and Immigration Services (USCIS) requires adjustment of status applicants to have a medical examination to show that they are free from health conditions that would make them inadmissible.

RECOMMENDED: What Happens at an Immigration Medical Exam

Purpose of the Medical Examination

The medical grounds of inadmissibility, the medical examination of foreign nationals, and the vaccinations administered to foreign nationals are designed to protect the health of the United States population. The immigration medical examination, the resulting medical examination report, and the vaccination record provide the information USCIS uses to determine if a foreign national meets the health-related standards for admissibility.

Four basic medical conditions may make an applicant inadmissible on health-related grounds:

  • Communicable disease of public health significance
  • An immigrant’s failure to show proof of required vaccinations
  • Physical or mental disorder with associated harmful behavior
  • Drug abuse or addiction

USCIS uses Form I-693 to gather results of a medical examination. The U.S. government requires the examination to establish that an applicant is not inadmissible to the United States on public health grounds.

RECOMMENDED: Grounds of Inadmissibility

Who Must Submit Form I-693

Almost every applicant for permanent residence must show that they are not inadmissible on the public health grounds. This does not apply to registry applicants and individuals born under diplomatic immunity in the United States. If you are filing Form I-485 under the nonimmigrant K-1/K-2 fiancé(e) or K-3/K-4 spouse categories and already had a medical examination overseas, you are not required to have another medical examination as long as you file your Form I-485 within one year of an overseas medical examination; and

  • The panel physician did not find a class A medical condition during your overseas examination; or
  • The panel physician did find a class A medical condition, you received a waiver of inadmissibility, and you have complied with the terms and conditions of the waiver.

Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will need to have the Part 10. Vaccination Record completed by a designated civil surgeon. In this case, you must submit Parts 1-5, 7, and 10 of Form I-693.

When to Submit Form I-693

Generally, it’s best to submit Form I-693 as part of the adjustment of status package. When you submit a complete application package, you make it easier for USCIS officers to give you a speedy approval. CitizenPath recommends that its customers submit the I-693 along with Form I-485, Application to Register Permanent Residence or Adjust Status . USCIS generally considers a completed Form I-693 to retain its evidentiary value for two years after the date the civil surgeon signed Form I-693.

However, USCIS does not currently require you to submit Form I-693 at the time you file Form I-485. Some people choose to submit the medical exam at the time of the in-person interview. While some applicants intend to bring their completed Form I-693 to their interview, some applications may not require an interview. USCIS makes decisions to waive interviews on a case-by-case basis. This would force USCIS to issue an RFE to request your medical exam results and would delay the progress of your case.

Mistakes on your I-485 application can cause costly delays or a denial.

The results of an immigration medical exam generally do not expire. Specifically, any Form I-693, Report of Immigration Medical Examination and Vaccination Record, that was properly completed and signed by a civil surgeon on or after November 1, 2023, does not expire and can be used indefinitely as evidence to show that the applicant is not inadmissible on health-related grounds.

RECOMMENDED: I-485 Checklist for Family-Based Applications

Only a USCIS designated civil surgeon can perform this medical examination in the United States. The civil surgeon must document the results of your medical examination on Form I-693. For more information on the medical examination, see the Form I-693 instructions .

I am working at Amazon currently. I filed I-485 in December 2020. But I did not include Medical reports along with my 485 application. Now I am thinking of sending the medicals. Did anyone send the medicals separately (not as a response for RFE)? Does Amazon support this? I have seen in some outside discussion forums that Amazon and Microsoft is encouraging 485 filers to send medicals soon (even though they did not get medical RFE). I did not get any email from immigration team to file my 693 (to interfile medicals). If you have filed your 693 recently please comment here. Thanks TC: 270K

USCIS urges eligible EB3 applicant to switch to EB2 or EB1 employment-based preference categories, because there is an exceptionally high number of employment-based immigrant visas available in these categories during this fiscal year (October 2021 through September 2022). Ref1 and Ref2 (see section “Transfer of Underlying Basis”)

Offer from USCIS to interfile I-485 is expired now but they still accept interfiling applications based on your case. Check with your attorney or take legal advice.

  • NO new I-485 application is required.

Only transfer requests accompanied by a Supplement J should be sent to the above address, and applicants should send no other forms, documents, or evidence to this address. However, USCIS will process written transfer requests submitted without a Supplement J sent to this address prior to March 4, 2022. Employment-based transfer requests that are not accompanied by a Supplement J should be submitted in writing to the USCIS office with jurisdiction over your pending I-485 application. Applicants may determine the USCIS office with jurisdiction over their application by referring to any receipt or transfer notices they have received, or by reaching out to the USCIS Contact Center at 800-375-5283. USCIS does not provide a written response to transfer requests. However, USCIS will issue receipt notices for the Supplement J.
