Millicent Elden Ring quest walkthrough
Cure the Scarlet Rot, and make a few friends along the way
by Mike Rougeau
Millicent’s Elden Ring questline is about a character you might never even meet if you don’t venture to a certain tucked-away spot in the rot-ridden region of Caelid .
Thankfully, we know exactly where Gowry’s Unalloyed Gold Needle is , how to unearth Sellia’s secrets , the cure for Scarlet Rot, where to meet Millicent next, and everything else about this quest — and we put it all together in this Elden Ring Millicent quest guide.
Table of contents
Chapter 1: sellia, town of sorcery, step 1: meeting gowry, step 2: getting the unalloyed gold needle.
- Step 3: Sellia’s secrets
Chapter 2: Meeting Millicent
Step 4: cure millicent, step 5: the journey begins, step 6: to altus plateau, step 7: windmill village, chapter 3: to the haligtree, step 8: the short version, step 9: a brief respite.
- Step 10: The Ulcerated Tree Spirit
Step 11: A choice
Step 12: a tragedy, final chapter: melenia.
The first leg of our journey to help Millicent takes us deep into Caelid , directly adjacent to the red swamp that you may have previously visited when you fell victim to a certain trapped chest in Limgrave’s Dragon-Burnt Ruins . You’re looking for a location called Sellia, Town of Sorcery , and, just to its south, a small shack that belongs to a certain sage named Gowry .
Gowry, who lives in a shack that looks like it’s from a Fallout game, requests that you recover a certain needle for him in exchange for telling you the secrets of Sellia. “What does this have to do with Millicent?” you might ask. Patience.
The needle can be found in the aforementioned rot swamp, known as Aeonia . Head to a large tangle of roots near its center, where you’ll find a boss named Commander O’Neil . The fight itself isn’t too tough — use a Spirit Ash to distract the Commander’s minions, and you’ll have him down in no time. In addition to his weapon, the Commander’s Standard , he’ll drop the Unalloyed Gold Needle that you need. Return it to the sage to begin the next leg of the journey.
Step 3: Sellia’s Secrets
Gowry gives you a slip of paper that, when inspected, reveals that you have to light three beacons around the Town of Sorcery to dispel the magical barriers blocking your progress.
Getting to these beacons involves heading to the opposite side of town and using Torrent’s double jump to climb around on roots and rooftops. It’s not a big place, so you shouldn’t have much trouble. You can see all three beacon steeples in the image below.
While you’re here, make sure to fully explore, as there’s decent loot to be found once the barriers are dispelled. Don’t miss the Golden Seed, Imbued Sword Key, “Redmane” painting puzzle, Spelldrake Talisman +1, and Lusat’s Glintstone Staff, which you get for defeating the boss through the fog gate on the northwest side of town.
Once you’re done, return to Gowry, who will send you on your way to give Millicent the magic needle.
Head to the north side of Sellia and out the back door, stopping at the Sellia Under-Stair site of grace. A quick trip uphill — dodging falling ball bearings — will have you on your way to the Church of the Plague , wherein resides the subject of our guide, Millicent.
The Church of the Plague itself is guarded by a pair of plague pests, nasty characters with whom you’re no doubt familiar, and the surrounding cemetery is filled with annoying, resurrecting skeletons. Run past the latter and bum rush the former, or take them out from a distance, to gain access to the church, which houses a site of grace and a valuable Sacred Tear for upgrading your flask.
Millicent lies moaning against the church’s western wall. Exhaust her dialogue, give her the Unalloyed Gold Needle, rest at the site of grace, and speak to her again. She’ll give you the Prosthesis-Wearer Heirloom , a talisman that raises Dexterity. Speak to her again until she starts repeating herself, and then rest at a site of grace and return to Gowry’s Shack.
At the shack, you’ll find Millicent, whose memory is beginning to return. Exhaust her dialogue, travel away, return one more time, and speak to Gowry. He’ll sell you some sorceries if you like.
You’ll next find Millicent standing by a cliff just north of the Erdtree-Gazing Hill site of grace . To progress her quest, you’ll need to visit Shaded Castle , a medium-sized dungeon located in the northern Altus Plateau . You’re looking for an item called the Valkyrie’s Prosthesis , found in a room guarded by a Cleanrot Knight on the castle’s northwest side.
Once you give Millicent the prosthesis, she’ll move on to her next location.
In the northeast region of Altus Plateau you’ll find Dominula, the Windmill Village . It’s extremely creepy. Murder the crazed inhabitants with wild abandon until you meet their leader, the Godskin Apostle , and defeat him to unlock the Windmill Heights site of grace. Rest here and Millicent will appear; exhaust her dialogue to proceed.
Millicent’s next locations are past the Capital, so if you haven’t completed the boss there yet, do so now. Furthermore, to continue Millicent’s quest, you’ll have to eventually reach Miquella’s Haligtree . If you have no idea what that is yet, then you’re probably not far enough into the game to worry about it yet anyway.
Here’s the short version: Get one half of the Haligtree Secret Medallion from Albus in the Albinauric Village , then meet Latenna in the Lakeside Crystal Cave . Then, once you’ve defeated Morgott and reached the Mountaintop of the Giants, meet Milllicent at the Ancient Snow Valley Ruins site of grace. Exhaust her dialogue, then continue north to Castle Sol and defeat the boss there. That will give you the other half of the medallion, which you can use at the Grand Lift of Rold to reach the path to the Haligtree.
Millicent’s next location is the Prayer Room grace in Elphael at the Haligtree. Be warned: This whole area is technically optional, and the enemies here are absolutely brutal. Even at high levels, you’ll have to tread carefully to make it through.
Once again, exhaust Millicent’s dialogue, then continue exploring Elphael.
Step 10: The Tree Spirit
On your way toward the Haligtree’s big boss, Melenia, you’ll encounter a large room full of plague pests and scarlet rot-inducing swamp water, just before the Drainage Channel site of grace. Cross a root over a gap in this room, and defeat the Ulcerated Tree Spirit that spawns there. Rest or fast travel to a site of grace, and then return to begin the next step.
When you return to the area where you fought the Tree Spirit, you’ll find two summon signs on the ground. Using the red sign will send you into Millicent’s world to fight her, and defeating her ends the questline and rewards you with the Millicent’s Prosthesis talisman, which raises dexterity and raises attack power with successive strikes.
On the other hand, if you use the gold summon sign, you’ll enter Millicent’s world as a friend to help her fend off four red phantom invaders known as Millicent’s Sisters . This rewards you with the Rotten Winged Sword Insignia , a talisman that “greatly” boosts attack power with successive strikes, and will allow you to continue Millicent’s quest.
We’re nearing the end here. Reload the area once more, and you’ll find Melenia nearby, once again suffering from scarlet rot. At this point, you have another chance to get the Millicent’s Prosthesis talisman by killing her outright. If instead you’d like to see the storyline through to the end, simply reload the area one last time, and you’ll find her corpse, along with Unalloyed Gold Needle from earlier.
To fully complete Millicent’s quest, you’ll need to proceed past the Drainage Channel to the Haligtree’s final area, and defeat Melenia. However, she’s easily the most difficult boss FromSoftware has ever designed. You’ll need every bit of skill and cunning to get past her insane flurries of attacks, which heal her with every strike. You may even need to respec your character with a Larval Tear if your build simply isn’t doing it.
Although it’s technically possible to beat Melenia using a number of different strategies, the vast majority of players simply lack the technical perfection needed to execute on anything but the most surefire methods. In short: summon the Mimic Tear Spirit Ash, and then spam Frost and Bleed effects at her, and hope for the best. Good luck.
Once you’ve defeated Melenia, you can use the Gold Needle on the scarlet blossom in her boss room to receive Miquella’s Needle , marking the official end of Millicent’s quest.
Now, what are you going to do with that? Well, it has something to do with another Elden Ring side quest — one we’ll have to get to another day.

Elden Ring Millicent Quest Guide: How to Get Rotten Winged Sword Insignia
Millicent is one of the many NPCs in the Lands Between with a personal storyline that contains a series of objectives. It can be tough to know exactly what to do to advance her questline as you progress, so we’ve got all the info you need right here. Here’s how to complete Millicent’s questline and get the Rotten Winged Sword Insignia in Elden Ring .
How to Complete Millicent Questline in Elden Ring
Millicent’s questline will likely be done as you progress through the game — not all at once — as it involves going to hard-to-reach areas that drastically jump up in difficulty. Here are the steps that you need to follow:
You’ll be summoned to help Millicent fight four of her sisters at once. If you choose to fight against Millicent, you’ll only get the Millicent’s Prosthesis Talisman as a reward.
You’ll receive the Rotten Winged Sword Insignia and Unalloyed Gold Needle after reloading the area. She’ll be gone and the items will be laying there on the ground.
What Does the Rotten Winged Sword Insignia Do?
This Talisman “greatly increases attack power with successive attacks,” which means that your combos will do more damage as the hits continue to pile on.
This is an especially useful effect for those who use the Hand of Malenia sword since its weapon skill is a flurry of strikes — just like the ones Malenia uses on you in her boss fight .
It’s a long questline, for sure — possibly one of the longest in the entire game — but that’s everything you need to know about how to complete Millicent’s quest and get the Rotten Winged Sword Insignia in Elden Ring .

The Rotten Winged Sword Insignia is one of the Talisman items in Elden Ring that can raise attack power when worn.
Where to Get the Rotten Winged Sword Insignia
The Rotten Winged Sword Insignia can be obtained by choosing to ally yourself with Milicent at the conclusion of her questline in Elphael, Brace of the Haligtree . After meeting her in the Prayer Room, defeat the nearby Ulcerated Tree Spirit and return to be able to invade the world of her sisters and help Milicent defeat them.
The Rotten Winged Sword Insignia will increase your attack power the longer you attack in a sustained manner, from 6%, to 8%, and then finally to 13%. If you do not keep attacking fast enough, the bonus will begin to decay until you attack again.

Elden Ring: The 15 Best Dexterity Weapons, Ranked
Elden ring: 11 lightest armor sets, ranked, hilarious elden ring glitch gives player's tarnished a goofy face whenever they do a certain move.
Elden Ring boasts well over 100 different talismans, each bearing useful bonus effects ranging from stat boosts to certain damage buffs and more. Like weapons and spells, there are also some legendary talismans with incredible buffs, such as the Radagon Icon, which reduces spell-casting speed, and the Godfrey Icon, amplifying charged attacks and spells. While legendary talismans are worthy of their status, regular talismans are nothing to overlook.
Players can discover plenty of powerful talismans on their journey through the Lands Between. Most damage-boosting talismans come with some downsides, but a few, like Millicent's Prosthesis, a Dexterity-boosting talisman that also increases attack after successive attacks, have no negative effects.
Unfortunately, Millicent’s Prosthesis can only be obtained by progressing through Milicent's questline . However, players need to be careful during Milicent’s questline in Elden Ring . If completed, players will be rewarded with the Rotten Winged Sword Insignia, a better version of Millicent’s Prosthesis but without extra Dexterity points.
While every weapon has its uses, some of the dexterity weapons in Elden Ring are a cut above the rest. These are the best ones and where to find them.
Elden Ring: Millicent's Prosthesis Talisman Location Guide
After Millicent reaches Erdtree-Gazing Hill in Altus Plateau in Elden Ring , players need to get her the Valkyrie’s Prosthesis to continue her questline. Then, head to Windmill Village north of Altus Plateau and defeat the Godskin Apostle boss to unlock the Windmill Heights grace site. Defeat Millicent at Windmill Heights to obtain Millicent’s Prosthesis in Elden Ring .
1) Defeat Commander O'Neil in the Swamp of Aeonia
In the center of Caelid, players can find a vast Scarlet Rot swamp housing a tough boss, Commander O'Neil. He can call forth spirit summons to assist in the fight against the Tarnished. If his summoned knights aren't dangerous enough, his Scarlet Rot attacks can be lethal. Commander O'Neil can also be lured into the rotted swamp, inflicting him with the Scarlet Rot effect. After defeating Commander O'Neil, players will receive a halberd called Commander’s Standard and the Unalloyed Gold Needle.
2) Bring the Unalloyed Gold Needle to Gowry
Next, bring the Unalloyed Gold Needle to Gowry, located in a shack south of Sellia, Town of Sorcery. He will take the needle and ask you to return later. Reload the game, and talk to Gowry to claim the repaired needle.
3) Give the Needle to Millicent in the Church of the Plague
Now, locate Millicent in the Church of the Plague, just above the cliff east of Gowry’s Shack. Give her the repaired needle to stop her crying and whimpering. Talk with her until you exhaust all of her dialogues. Then, rest at the grace site, and she should have moved to Gowry’s Shack. Go there and exhaust all her dialogues to make her leave for Altus Plateau.
4) Find Millicent at Erdtree-Gazing Hill and Give Her Valkyrie's Prosthesis
Go back to Millicent at Erdtree-Gazing Hill and give her the Valkyrie's Prosthesis. Exhaust all her dialogues to make her move to Windmill Village.
5) Beat Godskin Apostle at Dominula, Windmill Village
From the Outer Wall Battleground grace site, follow the path north to find the Windmill Village, the home of Celebrants . Reach the top of Windmill Village to face the Godskin Apostle boss. Defeat the Godskin Apostle to unlock the Windmill Heights grace site. If the boss is too difficult, you can summon Millicent to help in the fight.
After defeating the boss, rest at the grace site, and Millicent will appear south of the grace site. She can now use the Valkyrie’s Prosthesis as her right hand. You can defeat Millicent to obtain the Millicent’s Prosthesis talisman.
What Does Millicent’s Prosthesis Talisman do?
Millicent’s Prosthesis is a useful talisman for Dexterity builds in Elden Ring . It boosts the Tarnished’s Dexterity stat by 5 points and provides a notable attack buff when a nearby enemy is hit multiple times. The attack bonus depends on how many times you hit the enemy. After every three hits, Millicent’s Prosthesis talisman boosts the attack by 4%, followed by 6%, and finally by 11%.
It is a superior version of the Winged Sword Insignia found quite early in the game. Along with attack buffs from successive attacks, players also receive 5 Dexterity points, which can be helpful for low-level builds. Players planning to use the Millicent’s Prosthesis talisman are suggested to wield weapons with fast attacks, such as Claws, Straight Swords, Curved Swords, Twinblades, and so on. It also complements weapons with skills that hit multiple times, such as Hand of Malenia, Marais Executioner’s Sword, and Regalia of Eochaid.

Millicent's Prosthesis is arguably the "middle option" of these three (i.e. a Winged Sword Insignia +1). Though it also gives a straight-up stat boost of +5 Dexterity. This leads some to prefer Millicent's Prosthesis even over the Rotten Winged Sword Insignia — which provides the best damage boost but no extra stats.
Rotten winged sword insignia gives the stronger buff. Millicent's gives five extra dex and you can get it earlier. You can also wear MP with the regular winged sword insignia. But if you are going to wear two talismans RWSI has a stronger effect than the other two combined, and you can just wear the prosthesis wearers heirloom for the 5 dex.
One option is to just make do with the regular Winged Sword Insignia. You can get Millicent's Prosthesis without finishing her questline. Give her the Valkyrie's Prosthesis, kill the Godskin Apostle in the windmill village, and she should appear near where the apostle was. This is the earliest opportunity to kill her and get her prosthesis.
Rotten Winged Sword Insignia is a Talisman in Elden Ring . Rotten Winged Sword Insignia greatly raises attack power with successive attacks. Players can use talismans in Elden Ring to boost a variety of Stats. A talisman depicting a raised prosthetic blade. An honor bestowed upon the valkyries who serve the goddess of rot.
Elden Ring Millicent's Prosthesis Notes & Tips. Sell Value: 1,000; Can be equipped at the same time as the Prosthesis-Wearer Heirloom for a combined +10 to Dexterity, and the Winged Sword Insignia or Rotten Winged Sword Insignia to stack the attack buffs received from sequential attacks. To obtain Millicent's Prosthesis, start by speaking with Gowry in Caelid, retrieve the Valkyrie's Prosthesis from Shaded Castle, and deliver it to Millicent; for the Rotten Winged Sword Insignia, defeat the Cleanrot Knight in Stillwater Cave, then enhance it by interacting with Millicent post-gold needle exchange. Both talismans boost attack power significantly with successive hits, ideal for ...

Rotten would give me up to (rounded down), 378 attack power. Millicent's though would give me ~478, not including the attack power bonus. Tldr: Millicent's Prosthesis is better for a dex build than Rotten Sword Insignia. Depends entirely on your current level of Dex (soft/hard caps etc) and the Dex scaling of your weapon.
So I decided to test it myself. My Lvl 200 character (end of NG+) using Marais Executioner's to easily proc the bonuses. I found Rotten Sword insignia + Millicent's Prosthesis > Winged Sword Insignia + Prosthesis > Rotten Sword Insignia alone > Prosthesis alone. So in my opinion, if you only want to use 1 talisman pick Rotten Sword Insignia.
This rewards you with the Rotten Winged Sword Insignia, a talisman that "greatly" boosts attack power with successive strikes, and will allow you to continue Millicent's quest. Step 12: A ...

Considering both are bad for content that matters (bosses), the prostesis gives dex at least. So prostesis. What's good for bosses, currently stuck at Malenia lol. #5. Showing 1 - 5 of 5 comments. Per page: 15 30 50. ELDEN RING > General Discussions > Topic Details.
