The goal of a research proposal is twofold: to present and justify the need to study a research problem and to present the practical ways in which the proposed study should be conducted. The design elements and procedures for conducting research are governed by standards of the predominant discipline in which the problem resides, therefore, the guidelines for research proposals are more exacting and less formal than a general project proposal. Research proposals contain extensive literature reviews. They must provide persuasive evidence that a need exists for the proposed study. In addition to providing a rationale, a proposal describes detailed methodology for conducting the research consistent with requirements of the professional or academic field and a statement on anticipated outcomes and benefits derived from the study's completion.

How to Approach Writing a Research Proposal

Your professor may assign the task of writing a research proposal for the following reasons:

  • Develop your skills in thinking about and designing a comprehensive research study;
  • Learn how to conduct a comprehensive review of the literature to determine that the research problem has not been adequately addressed or has been answered ineffectively and, in so doing, become better at locating pertinent scholarship related to your topic;
  • Improve your general research and writing skills;
  • Practice identifying the logical steps that must be taken to accomplish one's research goals;
  • Critically review, examine, and consider the use of different methods for gathering and analyzing data related to the research problem; and,
  • Nurture a sense of inquisitiveness within yourself and to help see yourself as an active participant in the process of conducting scholarly research.

A proposal should contain all the key elements involved in designing a completed research study, with sufficient information that allows readers to assess the validity and usefulness of your proposed study. The only elements missing from a research proposal are the findings of the study and your analysis of those findings. Finally, an effective proposal is judged on the quality of your writing and, therefore, it is important that your proposal is coherent, clear, and compelling.

Regardless of the research problem you are investigating and the methodology you choose, all research proposals must address the following questions:

  • What do you plan to accomplish? Be clear and succinct in defining the research problem and what it is you are proposing to investigate.
  • Why do you want to do the research? In addition to detailing your research design, you also must conduct a thorough review of the literature and provide convincing evidence that it is a topic worthy of in-depth study. A successful research proposal must answer the "So What?" question.

Common Mistakes to Avoid

  • Failure to be concise . A research proposal must be focused and not be "all over the map" or diverge into unrelated tangents without a clear sense of purpose.
  • Failure to cite landmark works in your literature review . Proposals should be grounded in foundational research that lays a foundation for understanding the development and scope of the the topic and its relevance.
  • Failure to delimit the contextual scope of your research [e.g., time, place, people, etc.]. As with any research paper, your proposed study must inform the reader how and in what ways the study will frame the problem.
  • Failure to develop a coherent and persuasive argument for the proposed research . This is critical. In many workplace settings, the research proposal is a formal document intended to argue for why a study should be funded.
  • Sloppy or imprecise writing, or poor grammar . Although a research proposal does not represent a completed research study, there is still an expectation that it is well-written and follows the style and rules of good academic writing.
  • Too much detail on minor issues, but not enough detail on major issues . Your proposal should focus on only a few key research questions in order to support the argument that the research needs to be conducted. Minor issues, even if valid, can be mentioned but they should not dominate the overall narrative.

Structure and Writing Style

Beginning the Proposal Process

As with writing most college-level academic papers, research proposals are generally organized the same way throughout most social science disciplines. The text of proposals generally vary in length between ten and thirty-five pages, followed by the list of references. However, before you begin, read the assignment carefully and, if anything seems unclear, ask your professor whether there are any specific requirements for organizing and writing the proposal.

A good place to begin is to ask yourself a series of questions:

  • What do I want to study?
  • Why is the topic important?
  • How is it significant within the subject areas covered in my class?
  • What problems will it help solve?
  • How does it build upon [and hopefully go beyond] research already conducted on the topic?
  • What exactly should I plan to do, and can I get it done in the time available?

In general, a compelling research proposal should document your knowledge of the topic and demonstrate your enthusiasm for conducting the study. Approach it with the intention of leaving your readers feeling like, "Wow, that's an exciting idea and I can’t wait to see how it turns out!"

Most proposals should include the following sections:

I.  Introduction

In the real world of higher education, a research proposal is most often written by scholars seeking grant funding for a research project or it's the first step in getting approval to write a doctoral dissertation. Even if this is just a course assignment, treat your introduction as the initial pitch of an idea based on a thorough examination of the significance of a research problem. After reading the introduction, your readers should not only have an understanding of what you want to do, but they should also be able to gain a sense of your passion for the topic and to be excited about the study's possible outcomes. Note that most proposals do not include an abstract [summary] before the introduction.

Think about your introduction as a narrative written in two to four paragraphs that succinctly answers the following four questions :

  • What is the central research problem?
  • What is the topic of study related to that research problem?
  • What methods should be used to analyze the research problem?
  • Answer the "So What?" question by explaining why this is important research, what is its significance, and why should someone reading the proposal care about the outcomes of the proposed study?

II.  Background and Significance

This is where you explain the scope and context of your proposal and describe in detail why it's important. It can be melded into your introduction or you can create a separate section to help with the organization and narrative flow of your proposal. Approach writing this section with the thought that you can’t assume your readers will know as much about the research problem as you do. Note that this section is not an essay going over everything you have learned about the topic; instead, you must choose what is most relevant in explaining the aims of your research.

To that end, while there are no prescribed rules for establishing the significance of your proposed study, you should attempt to address some or all of the following:

  • State the research problem and give a more detailed explanation about the purpose of the study than what you stated in the introduction. This is particularly important if the problem is complex or multifaceted .
  • Present the rationale of your proposed study and clearly indicate why it is worth doing; be sure to answer the "So What? question [i.e., why should anyone care?].
  • Describe the major issues or problems examined by your research. This can be in the form of questions to be addressed. Be sure to note how your proposed study builds on previous assumptions about the research problem.
  • Explain the methods you plan to use for conducting your research. Clearly identify the key sources you intend to use and explain how they will contribute to your analysis of the topic.
  • Describe the boundaries of your proposed research in order to provide a clear focus. Where appropriate, state not only what you plan to study, but what aspects of the research problem will be excluded from the study.
  • If necessary, provide definitions of key concepts, theories, or terms.

III.  Literature Review

Connected to the background and significance of your study is a section of your proposal devoted to a more deliberate review and synthesis of prior studies related to the research problem under investigation . The purpose here is to place your project within the larger whole of what is currently being explored, while at the same time, demonstrating to your readers that your work is original and innovative. Think about what questions other researchers have asked, what methodological approaches they have used, and what is your understanding of their findings and, when stated, their recommendations. Also pay attention to any suggestions for further research.

Since a literature review is information dense, it is crucial that this section is intelligently structured to enable a reader to grasp the key arguments underpinning your proposed study in relation to the arguments put forth by other researchers. A good strategy is to break the literature into "conceptual categories" [themes] rather than systematically or chronologically describing groups of materials one at a time. Note that conceptual categories generally reveal themselves after you have read most of the pertinent literature on your topic so adding new categories is an on-going process of discovery as you review more studies. How do you know you've covered the key conceptual categories underlying the research literature? Generally, you can have confidence that all of the significant conceptual categories have been identified if you start to see repetition in the conclusions or recommendations that are being made.

NOTE: Do not shy away from challenging the conclusions made in prior research as a basis for supporting the need for your proposal. Assess what you believe is missing and state how previous research has failed to adequately examine the issue that your study addresses. To help frame your proposal's review of prior research, consider the "five C’s" of writing a literature review:

  • Cite , so as to keep the primary focus on the literature pertinent to your research problem.
  • Compare the various arguments, theories, methodologies, and findings expressed in the literature: what do the authors agree on? Who applies similar approaches to analyzing the research problem?
  • Contrast the various arguments, themes, methodologies, approaches, and controversies expressed in the literature: describe what are the major areas of disagreement, controversy, or debate among scholars?
  • Critique the literature: Which arguments are more persuasive, and why? Which approaches, findings, and methodologies seem most reliable, valid, or appropriate, and why? Pay attention to the verbs you use to describe what an author says/does [e.g., asserts, demonstrates, argues, etc.].
  • Connect the literature to your own area of research and investigation: how does your own work draw upon, depart from, synthesize, or add a new perspective to what has been said in the literature?

IV.  Research Design and Methods

This section must be well-written and logically organized because you are not actually doing the research, yet, your reader must have confidence that you have a plan worth pursuing . The reader will never have a study outcome from which to evaluate whether your methodological choices were the correct ones. Thus, the objective here is to convince the reader that your overall research design and proposed methods of analysis will correctly address the problem and that the methods will provide the means to effectively interpret the potential results. Your design and methods should be unmistakably tied to the specific aims of your study.

Describe the overall research design by building upon and drawing examples from your review of the literature. Consider not only methods that other researchers have used, but methods of data gathering that have not been used but perhaps could be. Be specific about the methodological approaches you plan to undertake to obtain information, the techniques you would use to analyze the data, and the tests of external validity to which you commit yourself [i.e., the trustworthiness by which you can generalize from your study to other people, places, events, and/or periods of time].

When describing the methods you will use, be sure to cover the following:

  • Specify the research process you will undertake and the way you will interpret the results obtained in relation to the research problem. Don't just describe what you intend to achieve from applying the methods you choose, but state how you will spend your time while applying these methods [e.g., coding text from interviews to find statements about the need to change school curriculum; running a regression to determine if there is a relationship between campaign advertising on social media sites and election outcomes in Europe ].
  • Keep in mind that the methodology is not just a list of tasks; it is a deliberate argument as to why techniques for gathering information add up to the best way to investigate the research problem. This is an important point because the mere listing of tasks to be performed does not demonstrate that, collectively, they effectively address the research problem. Be sure you clearly explain this.
  • Anticipate and acknowledge any potential barriers and pitfalls in carrying out your research design and explain how you plan to address them. No method applied to research in the social and behavioral sciences is perfect, so you need to describe where you believe challenges may exist in obtaining data or accessing information. It's always better to acknowledge this than to have it brought up by your professor!

V.  Preliminary Suppositions and Implications

Just because you don't have to actually conduct the study and analyze the results, doesn't mean you can skip talking about the analytical process and potential implications . The purpose of this section is to argue how and in what ways you believe your research will refine, revise, or extend existing knowledge in the subject area under investigation. Depending on the aims and objectives of your study, describe how the anticipated results will impact future scholarly research, theory, practice, forms of interventions, or policy making. Note that such discussions may have either substantive [a potential new policy], theoretical [a potential new understanding], or methodological [a potential new way of analyzing] significance.   When thinking about the potential implications of your study, ask the following questions:

  • What might the results mean in regards to challenging the theoretical framework and underlying assumptions that support the study?
  • What suggestions for subsequent research could arise from the potential outcomes of the study?
  • What will the results mean to practitioners in the natural settings of their workplace, organization, or community?
  • Will the results influence programs, methods, and/or forms of intervention?
  • How might the results contribute to the solution of social, economic, or other types of problems?
  • Will the results influence policy decisions?
  • In what way do individuals or groups benefit should your study be pursued?
  • What will be improved or changed as a result of the proposed research?
  • How will the results of the study be implemented and what innovations or transformative insights could emerge from the process of implementation?

NOTE:   This section should not delve into idle speculation, opinion, or be formulated on the basis of unclear evidence . The purpose is to reflect upon gaps or understudied areas of the current literature and describe how your proposed research contributes to a new understanding of the research problem should the study be implemented as designed.

ANOTHER NOTE : This section is also where you describe any potential limitations to your proposed study. While it is impossible to highlight all potential limitations because the study has yet to be conducted, you still must tell the reader where and in what form impediments may arise and how you plan to address them.

VI.  Conclusion

The conclusion reiterates the importance or significance of your proposal and provides a brief summary of the entire study . This section should be only one or two paragraphs long, emphasizing why the research problem is worth investigating, why your research study is unique, and how it should advance existing knowledge.

Someone reading this section should come away with an understanding of:

  • Why the study should be done;
  • The specific purpose of the study and the research questions it attempts to answer;
  • The decision for why the research design and methods used where chosen over other options;
  • The potential implications emerging from your proposed study of the research problem; and
  • A sense of how your study fits within the broader scholarship about the research problem.

VII.  Citations

As with any scholarly research paper, you must cite the sources you used . In a standard research proposal, this section can take two forms, so consult with your professor about which one is preferred.

  • References -- a list of only the sources you actually used in creating your proposal.
  • Bibliography -- a list of everything you used in creating your proposal, along with additional citations to any key sources relevant to understanding the research problem.

In either case, this section should testify to the fact that you did enough preparatory work to ensure the project will complement and not just duplicate the efforts of other researchers. It demonstrates to the reader that you have a thorough understanding of prior research on the topic.

Most proposal formats have you start a new page and use the heading "References" or "Bibliography" centered at the top of the page. Cited works should always use a standard format that follows the writing style advised by the discipline of your course [e.g., education=APA; history=Chicago] or that is preferred by your professor. This section normally does not count towards the total page length of your research proposal.

Whether you’re nearing the end of your degree and your dissertation is on the horizon, or you’re planning to apply for a PhD program, chances are you’ll need to craft a convincing research proposal . If you’re on this page, you’re probably unsure exactly what the research proposal is all about. Well, you’ve come to the right place.

What is a research proposal?

Simply put, a research proposal is a structured, formal document that explains what you plan to research (your research topic), why it’s worth researching (your justification), and how  you plan to investigate it (your methodology). 

The purpose of the research proposal (its job, so to speak) is to convince  your research supervisor, committee or university that your research is  suitable  (for the requirements of the degree program) and  manageable  (given the time and resource constraints you will face). 

The most important word here is “ convince ” – in other words, your research proposal needs to  sell  your research idea (to whoever is going to approve it). If it doesn’t convince them (of its suitability and manageability), you’ll need to revise and resubmit . This will cost you valuable time, which will either delay the start of your research or eat into its time allowance (which is bad news). 

A research proposal is a  formal document that explains what you plan to research , why it's worth researching and how you'll do it.

What goes into a research proposal?

A good dissertation or thesis proposal needs to cover the “ what “, “ why ” and” how ” of the proposed study. Let’s look at each of these attributes in a little more detail:

Your proposal needs to clearly articulate your research topic . This needs to be specific and unambiguous . Your research topic should make it clear exactly what you plan to research and in what context. Here’s an example of a well-articulated research topic:

An investigation into the factors which impact female Generation Y consumer’s likelihood to promote a specific makeup brand to their peers: a British context

As you can see, this topic is extremely clear. From this one line we can see exactly:

  • What’s being investigated – factors that make people promote or advocate for a brand of a specific makeup brand
  • Who it involves – female Gen-Y consumers
  • In what context – the United Kingdom

So, make sure that your research proposal provides a detailed explanation of your research topic . If possible, also briefly outline your research aims and objectives , and perhaps even your research questions (although in some cases you’ll only develop these at a later stage). Needless to say, don’t start writing your proposal until you have a clear topic in mind , or you’ll end up waffling and your research proposal will suffer as a result of this.

As we touched on earlier, it’s not good enough to simply propose a research topic – you need to justify why your topic is original . In other words, what makes it  unique ? What gap in the current literature does it fill? If it’s simply a rehash of the existing research, it’s probably not going to get approval – it needs to be fresh.

But,  originality  alone is not enough. Once you’ve ticked that box, you also need to justify why your proposed topic is  important . In other words, what value will it add to the world if you achieve your research aims?

As an example, let’s look at the sample research topic we mentioned earlier (factors impacting brand advocacy). In this case, if the research could uncover relevant factors, these findings would be very useful to marketers in the cosmetics industry, and would, therefore, have commercial value . That is a clear justification for the research.

So, when you’re crafting your research proposal, remember that it’s not enough for a topic to simply be unique. It needs to be useful and value-creating – and you need to convey that value in your proposal. It’s all good and well to have a great topic that’s original and valuable, but you’re not going to convince anyone to approve it without discussing the practicalities – in other words:

  • How will you actually undertake your research (i.e., your methodology)?
  • Is your research methodology appropriate given your research aims?
  • Is your approach manageable given your constraints (time, money, etc.)?

While it’s generally not expected that you’ll have a fully fleshed-out methodology at the proposal stage, you’ll likely still need to provide a high-level overview of your research methodology . Here are some important questions you’ll need to address in your research proposal:

  • Will you take a qualitative , quantitative or mixed -method approach?
  • What sampling strategy will you adopt?
  • How will you collect your data (e.g., interviews, surveys, etc)?
  • How will you analyse your data (e.g., descriptive and inferential statistics , content analysis, discourse analysis, etc, .)?
  • What potential limitations will your methodology carry?

So, be sure to give some thought to the practicalities of your research and have at least a basic methodological plan before you start writing up your proposal. How To Structure A Research Proposal

Now that we’ve covered the key points that need to be addressed in a proposal, you may be wondering, “ But how is a research proposal structured? “.

While the exact structure and format required for a research proposal differs from university to university, there are four “essential ingredients” that commonly make up the structure of a research proposal:

  • A rich introduction and background to the proposed research
  • An initial literature review covering the existing research
  • An overview of the proposed research methodology
  • A discussion regarding the practicalities (project plans, timelines, etc.)

How long should a research proposal be.

This varies tremendously, depending on the university, the field of study (e.g., social sciences vs natural sciences), and the level of the degree (e.g. undergraduate, Masters or PhD) – so it’s always best to check with your university what their specific requirements are before you start planning your proposal.

As a rough guide, a formal research proposal at Masters-level often ranges between 2000-3000 words, while a PhD-level proposal can be far more detailed, ranging from 5000-8000 words. In some cases, a rough outline of the topic is all that’s needed, while in other cases, universities expect a very detailed proposal that essentially forms the first three chapters of the dissertation or thesis.

Addressing ethical issues in your research proposal

This article explores the ethical issues that may arise in your proposed study during your doctoral research degree.

What ethical principles apply when planning and conducting research?

Research ethics are the moral principles that govern how researchers conduct their studies (Wellcome Trust, 2014). As there are elements of uncertainty and risk involved in any study, every researcher has to consider how they can uphold these ethical principles and conduct the research in a way that protects the interests and welfare of participants and other stakeholders (such as organisations).  

You will need to consider the ethical issues that might arise in your proposed study. Consideration of the fundamental ethical principles that underpin all research will help you to identify the key issues and how these could be addressed. As you are probably a practitioner who wants to undertake research within your workplace, consider how your role as an ‘insider’ influences how you will conduct your study. Think about the ethical issues that might arise when you become an insider researcher (for example, relating to trust, confidentiality and anonymity).  

What key ethical principles do you think will be important when planning or conducting your research, particularly as an insider? Principles that come to mind might include autonomy, respect, dignity, privacy, informed consent and confidentiality. You may also have identified principles such as competence, integrity, wellbeing, justice and non-discrimination.  

Key ethical issues that you will address as an insider researcher include:

  • Gaining trust
  • Avoiding coercion when recruiting colleagues or other participants (such as students or service users)
  • Practical challenges relating to ensuring the confidentiality and anonymity of organisations and staff or other participants.

Ethical principles are essential for protecting the interests of research participants, including maximising the benefits and minimising any risks associated with taking part in a study. These principles describe ethical conduct which reflects the integrity of the researcher, promotes the wellbeing of participants and ensures high-quality research is conducted (Health Research Authority, 2022).  

Research ethics is therefore not simply about gaining ethical approval for your study to be conducted. Research ethics relates to your moral conduct as a doctoral researcher and will apply throughout your study from design to dissemination (British Psychological Society, 2021). When you apply to undertake a doctorate, you will need to clearly indicate in your proposal that you understand these ethical principles and are committed to upholding them.  

Where can I find ethical guidance and resources? 

Professional bodies, learned societies, health and social care authorities, academic publications, Research Ethics Committees and research organisations provide a range of ethical guidance and resources. International codes such as the Universal Declaration of Human Rights underpin ethical frameworks (United Nations, 1948).  

You may be aware of key legislation in your own country or the country where you plan to undertake the research, including laws relating to consent, data protection and decision-making capacity, for example, the Data Protection Act, 2018 (UK).  If you want to find out more about becoming an ethical researcher, check out this Open University short course: Becoming an ethical researcher: Introduction and guidance: What is a badged course? - OpenLearn - Open University  

You should be able to justify the research decisions you make. Utilising these resources will guide your ethical judgements when writing your proposal and ultimately when designing and conducting your research study. The Ethical Guidelines for Educational Research (British Educational Research Association, 2018) identifies the key responsibilities you will have when you conduct your research, including the range of stakeholders that you will have responsibilities to, as follows:   

  • to your participants (e.g. to appropriately inform them, facilitate their participation and support them)
  • clients, stakeholders and sponsors
  • the community of educational or health and social care researchers
  • for publication and dissemination
  • your wellbeing and development

The National Institute for Health and Care Research (no date) has emphasised the need to promote equality, diversity and inclusion when undertaking research, particularly to address long-standing social and health inequalities. Research should be informed by the diversity of people’s experiences and insights, so that it will lead to the development of practice that addresses genuine need. A commitment to equality, diversity and inclusion aims to eradicate prejudice and discrimination on the basis of an individual or group of individuals' protected characteristics such as sex (gender), disability, race, sexual orientation, in line with the Equality Act 2010.  

The NIHR has produced guidance for enhancing the inclusion of ‘under-served groups’ when designing a research study (2020). Although the guidance refers to clinical research it is relevant to research more broadly.  

You should consider how you will promote equality and diversity in your planned study, including through aspects such as your research topic or question, the methodology you will use, the participants you plan to recruit and how you will analyse and interpret your data.    

What ethical issues do I need to consider when writing my research proposal?

You might be planning to undertake research in a health, social care, educational or other setting, including observations and interviews. The following prompts should help you to identify key ethical issues that you need to bear in mind when undertaking research in such settings.  

1.     Imagine you are a potential participant. Think about the questions and concerns that you might have:

  • How would you feel if a researcher sat in your space and took notes, completed a checklist, or made an audio or film recording?
  • What harm might a researcher cause by observing or interviewing you and others?
  • What would you want to know about the researcher and ask them about the study before giving consent?
  • When imagining you are the participant, how could the researcher make you feel more comfortable to be observed or interviewed? 

2.     Having considered the perspective of your potential participant, how would you take account of concerns such as privacy, consent, wellbeing and power in your research proposal?

The ethical issues to be considered will vary depending on your organisational context/role, the types of participants you plan to recruit (for example, children, adults with mental health problems), the research methods you will use, and the types of data you will collect. You will need to decide how to recruit your participants so you do not inappropriately exclude anyone.  Consider what methods may be necessary to facilitate their voice and how you can obtain their consent to taking part or ensure that consent is obtained from someone else as necessary, for example, a parent in the case of a child. 

You should also think about how to avoid imposing an unnecessary burden or costs on your participants. For example, by minimising the length of time they will have to commit to the study and by providing travel or other expenses. Identify the measures that you will take to store your participants’ data safely and maintain their confidentiality and anonymity when you report your findings. You could do this by storing interview and video recordings in a secure server and anonymising their names and those of their organisations using pseudonyms.  

Professional codes such as the Code of Human Research Ethics (BPS, 2021) provide guidance on undertaking research with children. Being an ‘insider’ researching within your own organisation has advantages. However, you should also consider how this might impact on your research, such as power dynamics, consent, potential bias and any conflict of interest between your professional and researcher roles (Sapiro and Matthews, 2020).  

How have other researchers addressed any ethical challenges?

The literature provides researchers’ accounts explaining how they addressed ethical challenges when undertaking studies. For example, Turcotte-Tremblay and McSween-Cadieux (2018) discuss strategies for protecting participants’ confidentiality when disseminating findings locally, such as undertaking fieldwork in multiple sites and providing findings in a generalised form. In addition, professional guidance includes case studies illustrating how ethical issues can be addressed, including when researching online forums (British Sociological Association, no date).

Here, you’ll need to outline specific, clear and achievable objectives that align with your research problem. These objectives should be well-defined, focused and measurable, serving as the guiding pillars for your study. They help in establishing what you intend to accomplish through your research and provide a clear direction for your investigation.

Step 4: Literature review

In this part, conduct a thorough review of existing literature related to your research topic. This involves a detailed summary of key findings and major contributions from previous research. Identify existing gaps in the literature and articulate how your research aims to fill these gaps. The literature review not only shows your grasp of the subject matter but also how your research will contribute new insights or perspectives to the field.

Step 5: Methodology

Here, in this section you should reassure reviewers that you are committed to conducting your research responsibly and ethically.

Step 9: Expected outcomes and significance

Articulate the expected outcomes or results of your research. Explain the potential impact and significance of these outcomes, whether in advancing academic knowledge, influencing policy or addressing specific societal or practical issues.

Compile a comprehensive list of all the references cited in your proposal. Adhere to a consistent citation style (like APA or MLA) throughout your document. Step 11: Appendices

A research proposal's purpose is to capture the evaluator's attention, demonstrate the study's potential benefits, and prove that it is a logical and consistent approach (Van Ekelenburg, 2010).  To ensure that your research proposal contains these elements, there are several aspects to include in your proposal (Al-Riyami, 2008):

  • Objective(s)
  • Variables (independent and dependent)
  • Research Question and/or hypothesis

Details about what to include in each element are included in the boxes below.  Depending on the topic of your study, some parts may not apply to your proposal.  You can also watch the video below for a brief overview about writing a successful research proposal.

Van Ekelenburg (2010) uses the PROSANA Model to guide researchers in developing rationale and justification for their research projects.  It is an acronym that connects the problem, solution, and benefits of a particular research project.  It is an easy way to remember the critical parts of a research proposal and how they relate to one another.  It includes the following letters (Van Ekelenburg, 2010):

  • Problem: Describing the main problem that the researcher is trying to solve.
  • Root causes: Describing what is causing the problem.  Why is the topic an issue?
  • fOcus: Narrowing down one of the underlying causes on which the researcher will focus for their research project.
  • Solutions: Listing potential solutions or approaches to fix to the problem.  There could be more than one.
  • Approach: Selecting the solution that the researcher will want to focus on.
  • Novelty: Describing how the solution will address or solve the problem.
  • Arguments: Explaining how the proposed solution will benefit the problem.

Research proposal titles should be concise and to the point, but informative.  The title of your proposal may be different from the title of your final research project, but that is completely normal!  Your findings may help you come up with a title that is more fitting for the final project.  Characteristics of good proposal titles are (Al-Riyami, 2008):

  • Catchy: It catches the reader's attention by peaking their interest.
  • Positive: It spins your project in a positive way towards the reader.
  • Transparent: It identifies the independent and dependent variables.

It is also common for proposal titles to be very similar to your research question, hypothesis, or thesis statement (Locke et al., 2007).

An abstract is a brief summary (about 300 words) of the study you are proposing.  It includes the following elements (Al-Riyami, 2008):

  • Your primary research question(s).
  • Hypothesis or main argument.
  • Method you will use to complete the study.  This may include the design, sample population, or measuring instruments that you plan to use.

The purpose of the introduction is to give readers background information about your topic.  it gives the readers a basic understanding of your topic so that they can further understand the significance of your proposal.  A good introduction will explain (Al-Riyami, 2008):

  • How it relates to other research done on the topic
  • Why your research is significant to the field
  • The relevance of your study

Your research objectives are the desired outcomes that you will achieve from the research project.  Depending on your research design, these may be generic or very specific.  You may also have more than one objective (Al-Riyami, 2008).

  • General objectives are what the research project will accomplish
  • Specific objectives relate to the research questions that the researcher aims to answer through the study.

Be careful not to have too many objectives in your proposal, as having too many can make your project lose focus.  Plus, it may not be possible to achieve several objectives in one study.

This section describes the different types of variables that you plan to have in your study and how you will measure them.  According to Al-Riyami (2008), there are four types of research variables:

  • Independent:  The person, object, or idea that is manipulated by the researcher.
  • Dependent:  The person, object, or idea whose changes are dependent upon the independent variable.  Typically, it is the item that the researcher is measuring for the study.
  • Confounding/Intervening:  Factors that may influence the effect of the independent variable on the dependent variable.  These include physical and mental barriers.  Not every study will have intervening variables, but they should be studied if applicable.
  • Background:   Factors that are relevant to the study's data and how it can be generalized.  Examples include demographic information such as age, sex, and ethnicity.

Your research proposal should describe each of your variables and how they relate to one another.  Depending on your study, you may not have all four types of variables present.  However, there will always be an independent and dependent variable.

A research question is the main piece of your research project because it explains what your study will discover to the reader.  It is the question that fuels the study, so it is important for it to be precise and unique.  You do not want it to be too broad, and it should identify a relationship between two variables (an independent and a dependent) (Al-Riyami, 2008).  There are six types of research questions (Academic Writer, n.d.):

  • Example: "Do people get nervous before speaking in front of an audience?"
  • Example: "What are the study habits of college freshmen at Tiffin University?"
  • Example: "What primary traits create a successful romantic relationship?"
  • Example: "Is there a relationship between a child's performance in school and their parents' socioeconomic status?"
  • Example: "Are high school seniors more motivated than high school freshmen?"
  • Example: "Do news media outlets impact a person's political opinions?"

If you know enough about your research topic that you believe a particular outcome may occur as a result of the study, you can include a hypothesis (thesis statement) in your proposal.  A hypothesis is a prediction that you believe will be the outcome of your study.  It explains what you think the relationship will be between the independent and dependent variable (Al-Riyami, 2008).  It is ok if the hypothesis in your proposal turns out to be incorrect, because it is only a prediction!  If you are writing a proposal in the humanities, you may be writing a thesis statement instead of a hypothesis.  A thesis presents the main argument of your research project and leads to corresponding evidence to support your argument.

Hypotheses vs. Theories

Hypotheses are different from theories in that theories represent general principles and sets of rules that explain different phenomena.  They typically represent large areas of study because they are applicable to anything in a particular field.  Hypotheses focus on specific areas within a field and are educated guesses, meaning that they have the potential to be proven wrong (Academic Writer, n.d.).  Because of this, hypotheses can also be formed from theories.

In a research proposal, you must thoroughly explain how you will conduct your study.  This includes things such as (Al-Riyami, 2008):

  • Research design:  What research approach will your study take?  Will it be quantitative or qualitative?
  • Research subjects/participants:  Who will be participating in your study?  Does your study require human participants?  How will you determine who to study?
  • Sample size:  How many participants will your study require?  If you are not using human participants, how much of the sample will you be studying?
  • Timeline:  A proposed list of the general tasks and events that you plan to complete the study.  This will include a time frame for each task/event and the order in which they will be completed.
  • Interventions:  If you plan on using anything on human participants for the study, you must include information it here.  This is especially important if you plan on using any substances on human subjects.
  • Ethical issues:  Are there any potential ethical issues surrounding this study?
  • Potential limitations:  Are there any limitations that could skew the data and findings from your study?
  • Appendixes:  If you need to present any consent forms, interview questions, surveys, questionnaires, or other items that will be used in your study, you should include samples of each item with an appendix to reference them.  If you are using a copyrighted document, you may need written permission from the original creator to use it in your study.  A copy of the written permission should be included in your proposal.
  • Setting:  Where will you be conducting the study?
  • Study instruments:  What measuring tools or computer software will you be using to collect data?  How will you collect the data?
  • How you will analyze the data:  What strategies or tools will you use to analyze the data you collect?
  • Quality control:  Will you have precautions in place to ensure that the study is conducted consistently and that outside factors will not skew the data?
  • Budget:  What type of funding will you need for your study?  This will include the funds needed to afford measuring tools, software, etc.
  • How you will share the study's findings:  What will you plan to do with the findings?
  • Significance of the study: How will your study expand on existing knowledge of the subject area?

The goal of a research proposal is to present and justify the need to study a research problem and to present the practical ways in which the proposed study should be conducted. The design elements and procedures for conducting the research are governed by standards within the predominant discipline in which the problem resides, so guidelines for research proposals are more exacting and less formal than a general project proposal. Research proposals contain extensive literature reviews. They must provide persuasive evidence that a need exists for the proposed study. In addition to providing a rationale, a proposal describes detailed methodology for conducting the research consistent with requirements of the professional or academic field and a statement on anticipated outcomes and/or benefits derived from the study's completion.

How to Approach Writing a Research Proposal

Your professor may assign the task of writing a research proposal for the following reasons:

  • Develop your skills in thinking about and designing a comprehensive research study;
  • Learn how to conduct a comprehensive review of the literature to ensure a research problem has not already been answered [or you may determine the problem has been answered ineffectively] and, in so doing, become better at locating scholarship related to your topic;
  • Improve your general research and writing skills;
  • Practice identifying the logical steps that must be taken to accomplish one's research goals;
  • Critically review, examine, and consider the use of different methods for gathering and analyzing data related to the research problem; and,
  • Nurture a sense of inquisitiveness within yourself and to help see yourself as an active participant in the process of doing scholarly research.

A proposal should contain all the key elements involved in designing a completed research study, with sufficient information that allows readers to assess the validity and usefulness of your proposed study. The only elements missing from a research proposal are the findings of the study and your analysis of those results. Finally, an effective proposal is judged on the quality of your writing and, therefore, it is important that your writing is coherent, clear, and compelling.

Regardless of the research problem you are investigating and the methodology you choose, all research proposals must address the following questions:

  • What do you plan to accomplish? Be clear and succinct in defining the research problem and what it is you are proposing to research.
  • Why do you want to do it? In addition to detailing your research design, you also must conduct a thorough review of the literature and provide convincing evidence that it is a topic worthy of study. Be sure to answer the "So What?" question.

Common Mistakes to Avoid

  • Failure to be concise; being "all over the map" without a clear sense of purpose.
  • Failure to cite landmark works in your literature review.
  • Failure to delimit the contextual boundaries of your research [e.g., time, place, people, etc.].
  • Failure to develop a coherent and persuasive argument for the proposed research.
  • Failure to stay focused on the research problem; going off on unrelated tangents.
  • Sloppy or imprecise writing, or poor grammar.
  • Too much detail on minor issues, but not enough detail on major issues.

Structure and Writing Style

Beginning the Proposal Process

As with writing a regular academic paper, research proposals are generally organized the same way throughout most social science disciplines. Proposals vary between ten and twenty-five pages in length. However, before you begin, read the assignment carefully and, if anything seems unclear, ask your professor whether there are any specific requirements for organizing and writing the proposal.

A good place to begin is to ask yourself a series of questions:

  • What do I want to study?
  • Why is the topic important?
  • How is it significant within the subject areas covered in my class?
  • What problems will it help solve?
  • How does it build upon [and hopefully go beyond] research already conducted on the topic?
  • What exactly should I plan to do, and can I get it done in the time available?

In general, a compelling research proposal should document your knowledge of the topic and demonstrate your enthusiasm for conducting the study. Approach it with the intention of leaving your readers feeling like--"Wow, that's an exciting idea and I can’t wait to see how it turns out!"

In general your proposal should include the following sections:

I.  Introduction

In the real world of higher education, a research proposal is most often written by scholars seeking grant funding for a research project or it's the first step in getting approval to write a doctoral dissertation. Even if this is just a course assignment, treat your introduction as the initial pitch of an idea or a thorough examination of the significance of a research problem. After reading the introduction, your readers should not only have an understanding of what you want to do, but they should also be able to gain a sense of your passion for the topic and be excited about the study's possible outcomes. Note that most proposals do not include an abstract [summary] before the introduction.

Think about your introduction as a narrative written in one to three paragraphs that succinctly answers the following four questions :

  • What is the central research problem?
  • What is the topic of study related to that problem?
  • What methods should be used to analyze the research problem?
  • Why is this important research, what is its significance, and why should someone reading the proposal care about the outcomes of the proposed study?

II.  Background and Significance

This section can be melded into your introduction or you can create a separate section to help with the organization and narrative flow of your proposal. This is where you explain the context of your proposal and describe in detail why it's important. Approach writing this section with the thought that you can’t assume your readers will know as much about the research problem as you do. Note that this section is not an essay going over everything you have learned about the topic; instead, you must choose what is relevant to help explain the goals for your study.

To that end, while there are no hard and fast rules, you should attempt to address some or all of the following key points:

  • State the research problem and give a more detailed explanation about the purpose of the study than what you stated in the introduction. This is particularly important if the problem is complex or multifaceted .
  • Present the rationale of your proposed study and clearly indicate why it is worth doing. Answer the "So What? question [i.e., why should anyone care].
  • Describe the major issues or problems to be addressed by your research. Be sure to note how your proposed study builds on previous assumptions about the research problem.
  • Explain how you plan to go about conducting your research. Clearly identify the key sources you intend to use and explain how they will contribute to your analysis of the topic.
  • Set the boundaries of your proposed research in order to provide a clear focus. Where appropriate, state not only what you will study, but what is excluded from the study.
  • If necessary, provide definitions of key concepts or terms.

III.  Literature Review

Connected to the background and significance of your study is a section of your proposal devoted to a more deliberate review and synthesis of prior studies related to the research problem under investigation . The purpose here is to place your project within the larger whole of what is currently being explored, while demonstrating to your readers that your work is original and innovative. Think about what questions other researchers have asked, what methods they have used, and what is your understanding of their findings and, where stated, their recommendations. Do not be afraid to challenge the conclusions of prior research. Since a literature review is information dense, it is crucial that this section is intelligently structured to enable a reader to grasp the key arguments underpinning your study in relation to that of other researchers. A good strategy is to break the literature into "conceptual categories" [themes] rather than systematically describing groups of materials one at a time. Note that conceptual categories generally reveal themselves after you have read most of the pertinent literature on your topic so adding new categories is an on-going process of discovery as you read more studies. How do you know you've covered the key conceptual categories underlying the research literature? Generally, you can have confidence that all of the significant conceptual categories have been identified if you start to see repetition in the conclusions or recommendations that are being made.

To help frame your proposal's literature review, here are the "five C’s" of writing a literature review:

  • Cite , so as to keep the primary focus on the literature pertinent to your research problem.
  • Compare the various arguments, theories, methodologies, and findings expressed in the literature: what do the authors agree on? Who applies similar approaches to analyzing the research problem?
  • Contrast the various arguments, themes, methodologies, approaches, and controversies expressed in the literature: what are the major areas of disagreement, controversy, or debate?
  • Critique the literature: Which arguments are more persuasive, and why? Which approaches, findings, methodologies seem most reliable, valid, or appropriate, and why? Pay attention to the verbs you use to describe what an author says/does [e.g., asserts, demonstrates, argues, etc.] .
  • Connect the literature to your own area of research and investigation: how does your own work draw upon, depart from, synthesize, or add a new perspective to what has been said in the literature?

IV.  Research Design and Methods

This section must be well-written and logically organized because you are not actually doing the research, yet, your reader must have confidence that it is worth pursuing . The reader will never have a study outcome from which to evaluate whether your methodological choices were the correct ones. Thus, the objective here is to convince the reader that your overall research design and methods of analysis will correctly address the problem and that the methods will provide the means to effectively interpret the potential results. Your design and methods should be unmistakably tied to the specific aims of your study.

Describe the overall research design by building upon and drawing examples from your review of the literature. Consider not only methods that other researchers have used but methods of data gathering that have not been used but perhaps could be. Be specific about the methodological approaches you plan to undertake to obtain information, the techniques you would use to analyze the data, and the tests of external validity to which you commit yourself [i.e., the trustworthiness by which you can generalize from your study to other people, places, events, and/or periods of time].

When describing the methods you will use, be sure to cover the following:

  • Specify the research operations you will undertake and the way you will interpret the results of these operations in relation to the research problem. Don't just describe what you intend to achieve from applying the methods you choose, but state how you will spend your time while applying these methods [e.g., coding text from interviews to find statements about the need to change school curriculum; running a regression to determine if there is a relationship between campaign advertising on social media sites and election outcomes in Europe ].
  • Keep in mind that a methodology is not just a list of tasks; it is an argument as to why these tasks add up to the best way to investigate the research problem. This is an important point because the mere listing of tasks to be performed does not demonstrate that, collectively, they effectively address the research problem. Be sure you explain this.
  • Anticipate and acknowledge any potential barriers and pitfalls in carrying out your research design and explain how you plan to address them. No method is perfect so you need to describe where you believe challenges may exist in obtaining data or accessing information. It's always better to acknowledge this than to have it brought up by your reader.

The goal of a research proposal is to present and justify a research idea you have and to present the practical ways in which you think this research should be conducted. The forms and procedures for such research are defined by the field of study, so guidelines for research proposals are generally more exacting and less formal than a project proposal. Research proposals contain extensive literature reviews and must provide persuasive evidence that there is a need for the research study being proposed. In addition to providing rationale for the proposed research, a proposal describes detailed methodology for conducting the research consistent with requirements of the professional or academic field and a statement on anticipated outcomes and/or benefits derived from the study.

How to Approach Writing a Research Proposal

Your professor may assign the task of writing a research proposal for the following reasons:

  • Develop your skills in thinking about and designing a comprehensive research study.
  • Help learn how to conduct a comprehensive review of the literature to ensure a research problem has not already been answered [or you may determine the problem has been answered ineffectively] and, in so doing, become familiar with scholarship related to your topic.
  • Improve your general research and writing skills.
  • Practice identifying what logical steps must be taken to accomplish one's research goals.
  • Nurture a sense of inquisitiveness within yourself and to help see yourself as an active participant in the process of doing scholarly research.

A proposal should contain all the key elements involved in designing a complete research study, with sufficient information that allows readers to assess the validity and usefulness of your proposed study. The only elements missing from a research proposal are the results of the study and your analysis of those results. Finally, an effective proposal is judged on the quality of your writing. It is, therefore, important that your writing is coherent, clear, and compelling.

Regardless of the research problem you are investigating and the methodology you choose, all research proposals must address the following questions:

  • What do you plan to accomplish? Be clear and succient in defining the research problem and what it is you are proposing to research.
  • Why do you want to do it? In addition to detailing your research design, you also must conduct a thorough review of the literature and provide convincing evidence that it is a topic worthy of study. Be sure to answer the "So what? question.
  • How are you going to do it? Be sure that what you propose is doable.

Common Mistakes to Avoid

  • Failure to be concise; being "all over the map" without a clear sense of purpose.
  • Failure to cite landmark works in your literature review.
  • Failure to delimit the contextual boundaries of your research [e.g., time, place, people, etc.].
  • Failure to develop a coherent and persuasive argument for the proposed research.
  • Failure to stay focused on the research question; going off on unrelated tangents.
  • Sloppy or imprecise writing. Poor grammar.
  • Too much detail on minor issues, but not enough detail on major issues.

Structure and Writing Style

Beginning the Proposal Process

As with writing a traditional research paper, research proposals are generally organized the same way throughout the social sciences. Most proposals are between ten and fifteen pages in length. However, before you begin, read the assignment carefully and, if anything seems unclear, ask your professor whether there are any specific requirements for organizing and writing the proposal.

A good place to begin is to ask yourself a series of questions:

  • What do I want to study, and why?
  • How is it significant within the subject areas covered in my class?
  • What problems will it help solve?
  • How does it build upon [and hopefully go beyond] research already conducted on my topic?
  • What exactly should I plan to do, and can I get it done in the time available?

In the end, your research proposal should document your knowledge of the topic and highlight enthusiasm for conducting the study. Approach it with the intention of leaving your readers feeling like--"Wow, that's an exciting idea and I can’t wait to see how it turns out!"

In general your proposal should include the following sections:

I.  Introduction

In the real world of higher education, a research proposal is most often written by scholars seeking grant funding for a research project or it's the first step in getting approval to write your doctoral dissertation. Even if this is just a course assignment, treat your introduction as the initial pitch of an idea. After reading the introduction, your readers should not only have an understanding of what you want to do, but they should also be able to sense your passion for the topic and be excited about its possible outcomes.

Think about your introduction as a narrative written in one to three paragraphs that succinctly answers the following four questions :

  • What is the central research problem?
  • What is the topic of study related to that problem?
  • What methods should be used to analyze the research problem?
  • Why is this important research, and why should someone reading the proposal care about the outcomes from the study?

II.  Background and Significance

This section can be melded into your introduction or you can create a separate section to help with the organization and flow of your proposal. This is where you explain the context of your project and outline why it's important. Approach writing this section with the thought that you can’t assume your readers will know as much about the research problem as you do. Note that this section is not an essay going over everything you have learned about the research problem; instead, you must choose what is relevant to help explain your goals for the study.

To that end, while there are no hard and fast rules, you should attempt to deal with some or all of the following:

  • State the research problem and give a more detailed explanation about the purpose of the study than what you stated in the introduction.
  • Present the rationale of your proposed study and clearly indicate why it is worth doing. Answer the "So what? question [i.e., why should anyone care].
  • Describe the major issues or problems to be addressed by your research.
  • Explain how you plan to go about conducting your research. Clearly identify the key sources you intend to use and explain how they will contribute to the analysis of your topic.
  • Set the boundaries of your proposed research in order to provide a clear focus.
  • Provide definitions of key concepts or terms, if necessary.

III.  Literature Review

Connected to the background and significance of your study is a more deliberate review and synthesis of prior studies related to the research problem under investigation . The purpose here is to place your project within the larger whole of what is currently being explored, while demonstrating to your readers that your work is original and innovative. Think about what questions other researchers have asked, what methods they've used, and what is your understanding of their findings. Assess what you believe is still missing, and state how previous research has failed to examine the issue that your study addresses.

Since a literature review is information dense, it is crucial that this section is intelligently structured to enable a reader to grasp the key arguments underpinning your study in relation to that of other researchers. A good strategy is to break the literature into "conceptual categories" [themes] rather than systematically describing materials one at a time.

To help frame your proposal's literature review, here are the "five C’s" of writing a literature review:

  • Cite : keep the primary focus on the literature pertinent to your research problem.
  • Compare the various arguments, theories, methodologies, and findings expressed in the literature: what do the authors agree on? Who applies similar approaches to analyzing the research problem?
  • Contrast the various arguments, themes, methodologies, approaches and controversies expressed in the literature: what are the major areas of disagreement, controversy, or debate?
  • Critique the literature: Which arguments are more persuasive, and why? Which approaches, findings, methodologies seem most reliable, valid, or appropriate, and why? Pay attention to the verbs you use to describe what an author says/does [e.g., asserts, demonstrates, etc.] .
  • Connect the literature to your own area of research and investigation: how does your own work draw upon, depart from, or synthesize what has been said in the literature?

IV.  Research Design and Methods

This section must be well-written and logically organized because you are not actually doing the research . As a consequence, the reader will never have a study outcome from which to evaluate whether your methodological choices were the correct ones. The objective here is to ensure that the reader is convinced that your overall research design and methods of analysis will correctly address the research problem. Your design and methods should be absolutely and unmistakably tied to the specific aims of your study.

Describe the overall research design by building upon and drawing examples from your review of the literature. Be specific about the methodological approaches you plan to undertake to collect information, about the techniques you will use to analyze it, and about tests of external validity to which you commit yourself [i.e., the trustworthiness by which you can generalize from your study to other people, places or times].

When describing the methods you will use, be sure to cover these issues:

  • Specify the research operations you will undertake and the way you will interpret the results of these operations in relation to your research problem. Don't just describe what you intend to achieve from applying the methods you choose, but state how you will spend your time while doing it.
  • Keep in mind that a methodology is not just a list of research tasks; it is an argument as to why these tasks add up to the best way to investigate the research problem. This is an important point because the mere listing of tasks to perform does not demonstrate that they add up to the best feasible approach.
  • Be sure to anticipate and acknowledge any potential barriers and pitfalls in carrying out your research design and explain how you plan to get around them.

V.  Preliminary Suppositions and Implications

Just because you don't have to actually conduct the study and analyze the results, it doesn't mean that you can skip talking about the process and potential implications . The purpose of this section is to argue how and in what ways you believe your research will refine, revise, or extend existing knowledge in the subject area under investigation. Depending on the aims and objectives of your study, describe how the anticipated results of your study will impact future scholarly research, theory, practice, forms of interventions, or policy. Note that such discussions may have either substantive [a potential new policy], theoretical [a potential new understanding], or methodological [a potential new way of analyzing] significance.   When thinking about the potential implications of your study, ask the following questions:

  • What might the results mean in regards to the theoretical framework that frames the study?
  • What suggestions for subsequent research could arise from the potential outcomes of the study?
  • What will the results mean to practitioners in the "real world"?
  • Will the results influence programs, methods, and/or forms of intervention?
  • How might the results contribute to the solution of social, economic, or other types of problems?
  • Will the results influence policy decisions?
  • What will be improved or changed as a result of the proposed research?
  • How will the results of the study be implemented, and what innovations will come about?

VI.  Conclusion

The conclusion reiterates the importance or significance of your proposal and provides a brief recap of the entire study . This section should be only one or two paragraphs long, emphasizing why your research study is unique, why it advances knowledge, and why the research problem is worth investigating.

Someone reading this section should come away with an understanding of:

  • Why the study was done,
  • The specific purpose of the study and the research questions it attempted to answer,
  • The research design and methods used,
  • The potential implications emerging from your proposed study of the research problem, and
  • A sense of how your study fits within the broader scholarship about the research problem.

VII.  Citations

As with any scholarly research paper, you must cite the sources you used in composing your proposal. In a standard research proposal, this section can take two forms, so speak with your professor about which one is preferred.

  • References -- lists only the literature that you actually used or cited in your proposal.
  • Bibliography -- lists everything you used or cited in your proposal with additional citations of any key sources relevant to understanding the research problem.

In either case, this section should testify to the fact that you did enough preparatory work to make sure the project will complement and not duplicate the efforts of other researchers. Start a new page and use the heading "References" or "Bibliography" at the top of the page. Cited works should always use a standard format that follows the writing style advised by the discipline of your course [i.e., education=APA; history=Chicago, etc]. This section normally does not count towards the total length of your proposal.

Drafting a Research Plan for IRB Review and Research Conduct: Information That Must Be Included in a Clinical Trial Protocol

A clinical trial is an interventional clinical study, in which participants are recruited for the administration of a specific intervention (use of a study drug, a procedure with a medical device, an educational intervention, etc.). Planning for a clinical research trial includes the development of a clinical protocol document. There are several reasons to have a clear and well-written protocol:

  • So that the research plan is clearly defined to everyone who will participate in conducting the study, and all processes and procedures are clearly and completely described to avoid variation in procedures and the introduction of assessment bias into the study conduct
  • So that oversight committees and agencies (Institutional Review Boards (IRBs)/Research Ethics Committees, scientific review committees, radiation safety or other committees, and regulatory agencies such as the Food and Drug Administration (FDA)) can review the research proposal in enough detail to ensure that it meets regulatory requirements to grant approval for the research conduct
  • So that the endpoints, study design, data collection parameters, and the data analysis plan are prospectively defined prior to the research being conducted, and bias cannot be introduced by changing the design or analysis plan during the study (for this reason, many journals now require public posting of the full research protocol along with the publication of study results).

There are many ways to outline and describe the necessary components of the research protocol and plan, and there are many protocol templates that are available to serve as guidelines for protocol development. In this paper, we are addressing these requirements from the perspective of the IRB. Federal regulations dictate specific criteria against which the IRB must review all protocol submissions , to make an independent determination regarding whether the prospective research plan can be approved. If the protocol (or other documents such as the IRB submission form and consent documents that accompany the protocol in the submission) does not provide adequate or complete information for the IRB to make a determination that all criteria have been met, the IRB cannot approve the research plan. This document is intended to assist investigators and research teams by providing a list of the necessary information that they can use as a writing guide, or use as a checklist against the protocol and submission document package, to ensure that all the necessary information is included prior to submitting the protocol and plan for review by an IRB.

TABLE 1 lists the criteria for the approval of research (paraphrased here from the full regulatory language in 21 CFR 56.111 ) by an IRB, and describes the information that is necessary for the clinical protocol (or elsewhere in the research plan submitted for review), for the IRB to be able to make determinations regarding whether a research plan can be approved.

TABLE 2 describes additional components or documents that may need to be included, based on specifics of the study product or the study plan.

In addition, all protocol documents should:

  • Be final documents. Draft protocols or other study documents should not be submitted for IRB review as the IRB must review final and complete study plans. If edits or changes are made after IRB review, the amended protocol must be resubmitted for approval of the changes.
  • Include page numbers and/or section or outline numbering so that the location of content can be identified
  • Include a table of contents
  • Include some kind of version control notation (either a version number or version date) so that revisions of the documents can be clearly tracked.

Criterion for approval of the research proposal:.

That risks to participants are minimized by using procedures that are consistent with sound research design and that do not unnecessarily expose subjects to risk, and whenever appropriate, by using procedures already being performed on the subjects for diagnostic or treatment purposes.

Corresponding Information required in the Protocol/Research Plan:

  • A clear and detailed list of all study procedures, including screening and follow-up procedures (often in tabular format, describing the procedures to be conducted at each study visit). “Procedures” includes (but is not limited to) study visits; interviews for the collection of safety or other data; vital sign measurements; surveys, questionnaires, or diary completion; study drug administration or other investigational study intervention; invasive and non-invasive procedures including blood draws, radiologic tests or other diagnostic testing.
  • The protocol should very clearly define what visits, assessments and procedures would be occurring as part of standard care
  • If participants need to leave the study early or suddenly, the protocol should include any specific testing or follow-up that may need to occur for safety purposes.

That risks to subjects are reasonable in relation to anticipated benefits, if any, to subjects, and the importance of the knowledge that may reasonably be expected to result.

  • the scientific knowledge expected to result from the research project
  • the potential direct benefits to individual research participants in the study
  • The protocol should also contain a clear and complete description of the study drugs/study intervention including (as appropriate) dosing information, storage information, and criteria for dose modification or dose adjustments.

That the selection of subjects is equitable.

  • Eligibility criteria should be designed to make the study as inclusive as possible while excluding persons with factors or conditions that unacceptably increase the potential risks, or unreasonably confound the measurement of study endpoints
  • Consider demographics, medical history and co-morbidities, concomitant medications, and whether the protocol must include testing for the presence or absence of conditions specified in the criteria
  • It is not necessary to specify opposite factors in both lists, i.e., if inclusion criterion is age ≥ 18, the exclusion does not need to specify age < 18.

Informed consent will be sought from each prospective subject or the subject’s legally authorized representative.

Informed consent will be appropriately documented or appropriately waived.

  • While the protocol submission must include a written informed consent document, it should also describe the process of obtaining informed consent from participants; how they will be identified and contacted, who will approach them about the study, and how they will be given adequate opportunity to ask questions during the consent discussion
  • The written informed consent document must provide all of the elements required by federal regulations
  • If potential participants may be incapable of providing informed consent and consent must be provided by legally authorized representatives, the consent process must describe this and the consent form must have appropriate signature spaces
  • While there are circumstances in which the regulations allow either a waiver of documentation of consent (no signature) or a waiver of obtaining consent for standard or for emergency research , if a waiver is requested the submission should describe how the proposed research meets the specific requirements for these waivers, and how participants’ rights and privacy will be protected if the waiver is granted.

When appropriate, the research plan makes adequate provisions for monitoring the data collected to ensure the safety of subjects.

  • The protocol should include an adequate description of the safety and efficacy data points that will be collected, including how adverse events will be collected, recorded, graded, and assessed
  • How will safety data be monitored on an ongoing basis to identify any unanticipated safety issues during the study? Who will review the data, and will the study include a Data Safety Monitoring Committee, Data Monitoring Committee, or Adjudication Committee?

When appropriate, there are adequate provisions to protect the privacy of subjects and to maintain the confidentiality of data.

  • The protocol or associated documents should describe how the privacy of participants will be protected (including if appropriate how people will be approached about the study and given the opportunity to ask questions in a private setting), as well as how the data being collected will be anonymized or de-identified, stored, transferred and analyzed to maintain confidentiality.

When some or all of the subjects are likely to be vulnerable to coercion or undue influence, such as children, prisoners, individuals with impaired decision making capacity, or economically or educationally disadvantaged persons, additional safeguards have been included in the study to protect the rights and welfare of these subjects.

  • Note that federal regulations have specific sections for additional precautions required in the research when research is conducted on children , pregnant persons , and prisoners . Inclusion or exclusion of these groups should be specified in the eligibility criteria.
  • Consideration should also be given to whether the protocol should include specific steps for the protection of participants who may be unable to make consent decisions (temporarily or permanently).

If the protocol is evaluating the safety and/or efficacy of a drug, biologic or medical device (even if the drug, biologic or medical device is already FDA approved).

Additional information or documents that must be provided:

The regulatory status of the drug/device must be provided to the IRB, either via

  • a description in the protocol,
  • the submission form, or
  • a copy of a letter from the FDA.

For drugs or biologics , the documentation must include an (Investigational New Drug (IND) number or a regulatory-based rationale for why the product is considered IND-exempt (21 CFR 312.2).

For medical devices , the documentation must specify whether the device is used ‘on label’ (for exactly the same indication and circumstances for which is has already been FDA-cleared or approved) or is investigational. If the device is investigational, the documentation must include an Investigational Device Exemption (IDE), an NSR justification, or a regulatory-based rationale for why the device is considered IDE-exempt.

When the protocol is evaluating interventions for foods , herbal products , dietary and other supplements, vitamins , and cosmetics , a regulatory-based justification for why that agent is not considered a drug or biologic as used in the research must be provided. Remember that foods that are being researched for possible use as drug products (looking for evidence of action in diagnosing, treating or mitigating a disease or condition) are regulated as drugs.

The IRB must ensure that research is being conducted in compliance with FDA regulations, so submitting appropriate documents or explanations may prevent questions during the review process. If the IRB cannot confirm documentation, or if the IRB is not completely sure that the FDA would agree with the justification for why certain research would not need FDA oversight (i.e., would be IND- or IDE- exempt), the IRB must ask the researcher/sponsor to consult with FDA and obtain documentation that FDA agrees that their oversight is not indicated.

Some or all of the subjects are likely to be vulnerable to coercion or undue influence, such as children, prisoners, pregnant persons, individuals with impaired decision-making capacity, or economically or educationally disadvantaged persons.

When persons from vulnerable populations may be enrolled in the research the research plan must always specify this and consideration should also be given to whether the protocol should include specific steps for the protection of participants who may be unable to make consent decisions (temporarily or permanently).

Note that federal regulations have specific sections for additional precautions required in the research when research is conducted on children , pregnant persons , and prisoners .

The study includes the use of surveys, questionnaires or other instruments (quality of life assessments, etc.).

If the protocol includes the use of surveys, all survey questions must be submitted as part of the protocol, an attachment to the protocol, or a supplemental document. If standardized questionnaires or instruments are being used, these must also be submitted. The IRB is required to review all participant-facing materials, including participant diaries and participant recruitment materials [note: some IRBs, including WCG IRB, do not require submission of common, standard instruments such as the SF-36, but if there is any doubt about whether submission is required it is better to add it and avoid possible delays].

Study participants must provide written informed consent.

The informed consent document must be provided for review and must have all the elements required by regulations .

A waiver of informed consent documentation is being requested.

In addition to the regulatory-based justification for the waiver (see Table 1), a Participant Information Sheet must be included in the submission.

A waiver or alteration of HIPAA requirements is being requested.

A regulatory-based justification must be provided specifying how the research is eligible for the waiver or alteration.

For researchers new at conducting clinical research or writing protocols, developing a new clinical trial protocol that is complete and sufficient for the purposes for which it will be needed is often a larger and more time-consuming project that was initially expected. While IRBs are generally happy to provide information and to answer specific regulatory questions in advance of a protocol being submitted, most do not have the resources or capacity to provide one-on-one guidance to researchers who have protocols that need major revisions to meet the criteria for approval.

Researchers may find it best to work with an experienced medical writer or clinical research consultant; while the cost of these services can be significant, this may be the most cost-effective plan when balanced against the time saved by the researcher and the facilitation of the review and approval process when guided by someone who is familiar with clinical research regulations.

Study Suggests Genetics as a Cause, Not Just a Risk, for Some Alzheimer’s

People with two copies of the gene variant APOE4 are almost certain to get Alzheimer’s, say researchers, who proposed a framework under which such patients could be diagnosed years before symptoms.

Scientists are proposing a new way of understanding the genetics of Alzheimer’s that would mean that up to a fifth of patients would be considered to have a genetically caused form of the disease.

Currently, the vast majority of Alzheimer’s cases do not have a clearly identified cause. The new designation, proposed in a study published Monday, could broaden the scope of efforts to develop treatments, including gene therapy, and affect the design of clinical trials.

It could also mean that hundreds of thousands of people in the United States alone could, if they chose, receive a diagnosis of Alzheimer’s before developing any symptoms of cognitive decline, although there currently are no treatments for people at that stage.

The new classification would make this type of Alzheimer’s one of the most common genetic disorders in the world, medical experts said.

“This reconceptualization that we’re proposing affects not a small minority of people,” said Dr. Juan Fortea, an author of the study and the director of the Sant Pau Memory Unit in Barcelona, Spain. “Sometimes we say that we don’t know the cause of Alzheimer’s disease,” but, he said, this would mean that about 15 to 20 percent of cases “can be tracked back to a cause, and the cause is in the genes.”

The idea involves a gene variant called APOE4. Scientists have long known that inheriting one copy of the variant increases the risk of developing Alzheimer’s, and that people with two copies, inherited from each parent, have vastly increased risk.

The new study , published in the journal Nature Medicine, analyzed data from over 500 people with two copies of APOE4, a significantly larger pool than in previous studies. The researchers found that almost all of those patients developed the biological pathology of Alzheimer’s, and the authors say that two copies of APOE4 should now be considered a cause of Alzheimer’s — not simply a risk factor.

The patients also developed Alzheimer’s pathology relatively young, the study found. By age 55, over 95 percent had biological markers associated with the disease. By 65, almost all had abnormal levels of a protein called amyloid that forms plaques in the brain, a hallmark of Alzheimer’s. And many started developing symptoms of cognitive decline at age 65, younger than most people without the APOE4 variant.

“The critical thing is that these individuals are often symptomatic 10 years earlier than other forms of Alzheimer’s disease,” said Dr. Reisa Sperling, a neurologist at Mass General Brigham in Boston and an author of the study.

She added, “By the time they are picked up and clinically diagnosed, because they’re often younger, they have more pathology.”

People with two copies, known as APOE4 homozygotes, make up 2 to 3 percent of the general population, but are an estimated 15 to 20 percent of people with Alzheimer’s dementia, experts said. People with one copy make up about 15 to 25 percent of the general population, and about 50 percent of Alzheimer’s dementia patients.

The most common variant is called APOE3, which seems to have a neutral effect on Alzheimer’s risk. About 75 percent of the general population has one copy of APOE3, and more than half of the general population has two copies.

Alzheimer’s experts not involved in the study said classifying the two-copy condition as genetically determined Alzheimer’s could have significant implications, including encouraging drug development beyond the field’s recent major focus on treatments that target and reduce amyloid.

Dr. Samuel Gandy, an Alzheimer’s researcher at Mount Sinai in New York, who was not involved in the study, said that patients with two copies of APOE4 faced much higher safety risks from anti-amyloid drugs.

When the Food and Drug Administration approved the anti-amyloid drug Leqembi last year, it required a black-box warning on the label saying that the medication can cause “serious and life-threatening events” such as swelling and bleeding in the brain, especially for people with two copies of APOE4. Some treatment centers decided not to offer Leqembi, an intravenous infusion, to such patients.

Dr. Gandy and other experts said that classifying these patients as having a distinct genetic form of Alzheimer’s would galvanize interest in developing drugs that are safe and effective for them and add urgency to current efforts to prevent cognitive decline in people who do not yet have symptoms.

“Rather than say we have nothing for you, let’s look for a trial,” Dr. Gandy said, adding that such patients should be included in trials at younger ages, given how early their pathology starts.

Besides trying to develop drugs, some researchers are exploring gene editing to transform APOE4 into a variant called APOE2, which appears to protect against Alzheimer’s. Another gene-therapy approach being studied involves injecting APOE2 into patients’ brains.

The new study had some limitations, including a lack of diversity that might make the findings less generalizable. Most patients in the study had European ancestry. While two copies of APOE4 also greatly increase Alzheimer’s risk in other ethnicities, the risk levels differ, said Dr. Michael Greicius, a neurologist at Stanford University School of Medicine who was not involved in the research.

“One important argument against their interpretation is that the risk of Alzheimer’s disease in APOE4 homozygotes varies substantially across different genetic ancestries,” said Dr. Greicius, who cowrote a study that found that white people with two copies of APOE4 had 13 times the risk of white people with two copies of APOE3, while Black people with two copies of APOE4 had 6.5 times the risk of Black people with two copies of APOE3.

“This has critical implications when counseling patients about their ancestry-informed genetic risk for Alzheimer’s disease,” he said, “and it also speaks to some yet-to-be-discovered genetics and biology that presumably drive this massive difference in risk.”

Under the current genetic understanding of Alzheimer’s, less than 2 percent of cases are considered genetically caused. Some of those patients inherited a mutation in one of three genes and can develop symptoms as early as their 30s or 40s. Others are people with Down syndrome, who have three copies of a chromosome containing a protein that often leads to what is called Down syndrome-associated Alzheimer’s disease .

Dr. Sperling said the genetic alterations in those cases are believed to fuel buildup of amyloid, while APOE4 is believed to interfere with clearing amyloid buildup.

Under the researchers’ proposal, having one copy of APOE4 would continue to be considered a risk factor, not enough to cause Alzheimer’s, Dr. Fortea said. It is unusual for diseases to follow that genetic pattern, called “semidominance,” with two copies of a variant causing the disease, but one copy only increasing risk, experts said.

The new recommendation will prompt questions about whether people should get tested to determine if they have the APOE4 variant.

Dr. Greicius said that until there were treatments for people with two copies of APOE4 or trials of therapies to prevent them from developing dementia, “My recommendation is if you don’t have symptoms, you should definitely not figure out your APOE status.”

He added, “It will only cause grief at this point.”

Finding ways to help these patients cannot come soon enough, Dr. Sperling said, adding, “These individuals are desperate, they’ve seen it in both of their parents often and really need therapies.”

The role of clinical breast examination (CBE) for early detection of breast cancer is extremely important in lower-middle-income countries (LMICs) where access to breast imaging is limited. Our study aimed to describe the outcomes of a community outreach breast education, home CBE and referral program for early recognition of breast abnormalities and improvement of breast cancer awareness in a rural district of Pakistan.

Eight health care workers (HCW) and a gynecologist were educated on basic breast cancer knowledge and trained to create breast cancer awareness and conduct CBE in the community. They were then deployed in the Dadu district of Pakistan where they carried out home visits to perform CBE in the community. Breast cancer awareness was assessed in the community using a standardized questionnaire and standard educational intervention was performed. Clinically detectable breast lesions were identified during home CBE and women were referred to the study gynecologist to confirm the presence of clinical abnormalities. Those confirmed to have clinical abnormalities were referred for imaging. Follow-up home visits were carried out to assess reasons for non-compliance in patients who did not follow-through with the gynecologist appointment or prescribed imaging and re-enforce the need for follow-up.

Basic breast cancer knowledge of HCWs and study gynecologist improved post-intervention. HCWs conducted home CBE in 8757 women. Of these, 149 were warranted a CBE by a physician (to avoid missing an abnormality), while 20 were found to have a definitive lump by HCWs, all were referred to the study gynecologist (CBE checkpoint). Only 50% (10/20) of those with a suspected lump complied with the referral to the gynecologist, where 90% concordance was found between their CBEs. Follow-up home visits were conducted in 119/169 non-compliant patients. Major reasons for non-compliance were a lack of understanding of the risks and financial constraints. A significant improvement was observed in the community’s breast cancer knowledge at the follow-up visits using the standardized post-test.

Conclusions

Basic and focused education of HCWs can increase their knowledge and dispel myths. Hand-on structured training can enable HCWs to perform CBE. Community awareness is essential for patient compliance and for early-detection, diagnosis, and treatment.

Introduction

Breast cancer is the most frequently diagnosed malignancy (barring skin cancers) and the fifth leading cause of cancer-related deaths worldwide [ 1 , 2 ]. According to GLOBOCAN 2022, 2.3 million new cases of breast cancer were diagnosed in 2022, with 666,103 patients dying from the disease [ 3 ]. Moreover, the incidence and mortality of breast cancer is expected to increase by 40% and 50% respectively by 2040 [ 3 ]. The rise in incidence is particularly steep in Asia, with these countries also seeing a significantly younger age of onset compared to the Western world [ 4 , 5 ]. In Pakistan, one in every nine women suffers from breast cancer, with the country having one of the highest incidence rates in the region (around 2.5 times higher than neighboring countries such as Iran and India) [ 6 , 7 ]. Breast cancer accounts for more than 20% of all malignancies in Pakistan, and almost half of all cancers in women [ 8 ].

The World Health Organization (WHO) has emphasized the role of early diagnosis of symptomatic breast cancer as a more feasible and economical strategy as compared to screening in resource-constrained countries [ 9 , 10 ]. Screening for breast cancer allows for detection of breast cancer at an earlier stage (especially when small enough to remain undetectable on clinical examination) and leads to significantly better management outcomes and less treatment expenditure [ 1 , 9 ]. While screening aims to identify lesions in asymptomatic and healthy individuals who have yet to develop clinical manifestations of disease, early detection of symptomatic breast cancer seeks to recognize individuals at an earlier stage than when they would otherwise present, allowing for more timely management and potentially better oncologic outcomes [ 9 ].

Early diagnosis and treatment are a cornerstone of efforts to reduce cancer-associated mortality in developed countries. In the United States (US), fewer than 20% of cancers present with advanced disease [ 11 ]. Data from Pakistan presents a stark contrast, with more than half of patients presenting with locally advanced or metastatic disease [ 11 ]. Mammography is the most effective screening modality for breast cancer in high-income countries. Multiple breast cancer screening trials have reported a reduction breast cancer-related mortality up to 25% among women undergoing mammography screening [ 12 ]. However, it remains under-utilized as a screening tool, both in developing and developed countries. Reasons for this range from misconceptions regarding screening methods, techniques, and radiation to lack of insurance or a care provider and fear of recall imaging, overdiagnosis leading to unnecessary biopsies and treatment and side effects [ 13 , 14 ]. In the US, more than 75% of eligible women are screened for breast cancer via mammography [ 15 ]. In a lower-middle-income country (LMIC) like Pakistan, access to investigations such as mammogram, breast ultrasound and needle biopsy is limited due to lack of availability of machines and trained personnel, lack of awareness and financial limitations (75% of healthcare financing in Pakistan is out-of-pocket and over one-third of the population lives below the poverty line) [ 16 , 17 ]. In addition, conservative sociocultural norms and religious factors also prevent women from seeking routine healthcare [ 18 ]. Given the lack of healthcare access coupled with a largely conservative culture, community outreach programs with home visits may be the ideal system for bringing initial breast cancer recognition home to the rural communities, enabling early confirmation of disease and initiation of treatment. Similar outreach programs have met with considerable success in other aspects of healthcare. These include programs improving screening and prevention of malaria, tuberculosis, and HIV and those targeting improvement maternal and neonatal mortality [ 19 , 20 ]. Thus, screening, and early detection interventions implemented in LMICs like Pakistan must take into account the local healthcare systems and social structures.

Clinical breast examination (CBE) is recommended as the preferred approach for early detection of symptomatic and clinically detectable breast cancer in LMICs such as Pakistan. It consists of inspection and palpation of the breasts and regional (axillary, supraclavicular, infraclavicular and cervical) lymph nodes of the patient in a sitting and supine position [ 21 ]. It can be readily performed by a primary care physician to identify abnormal breast findings and determine the need for further evaluation.[ 22 ], [ 23 ] In fact, while mammography is expected to miss over 20% of breast cancers, CBE is able to detect 3–45% of these false negative cases [ 24 , 25 , 26 ].

Due to the aforementioned sociocultural barriers towards mammographic breast screening in Pakistan, it is vital that early detection interventions employ more feasible methods such as CBE. Thus, the objective of this study was to describe the outcomes of a community outreach breast education, home CBE and referral program for early recognition of breast abnormalities and improvement of breast cancer awareness in a rural district of Pakistan. We conducted a community outreach and referral program where home CBE visits were conducted by trained healthcare workers (HCWs) for early detection of breast signs and symptoms, in a rural district of Pakistan. Women who had clinical abnormalities detected upon examination were then referred for further evaluation. During these visits, the women were also educated regarding breast cancer management. In this study, we report our results and experiences with this program. We believe that it is important to reinforce that early detection interventions for breast cancer may be implemented in LMICs like Pakistan using CBE as the preferred approach. Given that most patients with breast cancer present with advanced disease, CBE may be able to identify characteristic breast changes earlier and allow for timely treatment of the tumor at earlier stages [ 27 ].

Materials and methods

Study design and setting.

A quasi-experimental study was carried out over September 2021 - September 2022 in Sindh, Pakistan. The study team was primarily based at Aga Khan University (AKU) in Karachi, Sindh, while the field location where the community outreach program was implemented was situated in the Dadu district of Sindh, Pakistan. The Aga Khan University is an academic tertiary care private hospital and a health services agency of the Aga Khan Development Network (AKDN) in Pakistan. This study featured a collaboration between the Departments of Surgery and Maternal and Child Health at AKU. Ethical approval was obtained from the ethical review committee at AKU.

The Dadu district covers 19,070 km 2 in interior Sindh and is divided into four sub-divisions which are further divided into Union Councils (UC). The UC is the smallest administrative unit of Pakistan. The field location of our study consisted of five UCs within the Johi subdivision of the Dadu district. Some census data of the five included UCs, as collected by the AKU for local projects, are shown in Table  1 . Approximately 48.7% of the population is female. As of 2021, it has 47 Basic Health Units (BHU) and 5 Rural Health Centers (RHC) with a total of 503 beds (BHUs and RHCs are first-level primary healthcare facilities that serve rural populations). The doctor -to-patient ratio is 1:6,030, nurse-to-patient ratio is 1:39,629, and bed-to-patient ratio is 1:3,309 [ 28 ].

Study population and sample size calculation

The total population within the five target UCs was 64,023. Our target sub-population consisted of all adult women ≥ 18 years of age. Using an estimated 20% prevalence of abnormal CBE according to a similar study conducted in Tajikistan [ 29 ], 80% power, 95% confidence level, and design effect of 2, we calculated the minimum required sample size to be 547 individuals. This was inflated by 100% to mitigate against extreme rates of individuals being lost to follow-up, which we anticipated to be a significant real-world challenge, yielding a final minimum required sample size of 1,094. Cluster convenience sampling was used to identify women in the community.

Training workshop and outreach program

The study schema consisted of the following interventions in sequence as described:

Training of Health Care Workers (HCWs) : Non-physician HCWs received training at AKU, Karachi in September – October 2021. This specialized training program was designed to enhance HCWs’ skills in identifying suspicious breast problems, making appropriate and timely referrals, and improving general knowledge regarding breast cancer. This training was conducted and overseen by an attending breast surgeon at AKU. HCWs were taught how to perform clinical breast examinations (CBEs) and engaged in hands-on practice sessions with simulated breast disease models and real patients in clinics. In addition, the HCWs were educated regarding general knowledge regarding breast cancer, with special emphasis on treatment, evaluation and commonly held misconceptions among the public. Pre and post-intervention surveys were administered to evaluate improvement in knowledge.

Community outreach program with home visits : The HCWs were deployed into the community in the Johi subdivision in October 2021. The initial series of home visits took place between October 2021 to February 2022, with the HCWs performing home visits in groups of two. Each visit began with an introductory and informed consent-seeking debriefing, followed by CBE of all consenting adult women belonging to a household, an assessment of baseline breast cancer-related knowledge, and lastly, a brief, standardized educational intervention delivered verbally (Supplement). For each CBE performed, a checklist of examination findings was completed. In the event of any abnormal finding, a referral to a local gynecologist within Johi was made. All interactions during the home visits were conducted in the Sindhi language, which is the native language of the region.

Visit to the local gynecologist : Patients who complied with their referral (for a palpable breast concern) were evaluated by a gynecologist at the local District Health Quarter. The gynecologist repeated a CBE on all referred patients in order to validate the HCWs’ examination findings. All eligible patients were then referred for breast imaging, either mammography or ultrasound, to the nearest facility within Johi.

Follow-up home visits : The HCWs attempted to conduct follow-up home visits for all women who were non-compliant with initial referral to a gynecologist. These follow-up visits took place six months after the initial series of home visits. Patients were questioned as to the reasons for their non-compliance with referral using a self-designed structured questionnaire (Supplement: Sect.  4 ). In addition, the breast cancer-related knowledge survey was re-administered to the women to gauge improvement in knowledge since the educational intervention delivered at the initial home visits. Finally, the importance of complying with referral for future evaluation, diagnosis and management was re-emphasized to all patients.

Validation of Data Collection Tools :

CBE checklist : This was a self-designed checklist (Supplement: Sect.  3 ) that included all the important components of a CBE, including a brief history of relevant symptoms (pain, discharge), breast inspection (skin changes, or changes in breast size, shape, or symmetry, and nipple changes), and breast palpation (presence of lumps in the axilla or breast).

Breast cancer-related knowledge survey : Separate surveys were administered to the HCWs and the women within the general community (Supplement: Sects.  2 and 3 ). Both surveys were designed by faculty within the Section of Breast Surgery at AKU. Prior to its use, the survey for women within the community was pretested amongst 30 local women for content, comprehensibility, and language. Minor adjustments were made on the basis of this pilot procedure.

Statistical analysis

All analyses were performed using SPSS (Statistical Package for Social Sciences) version 23.0 (IBM Corporation, Armonk, New York). Descriptive analysis was performed whereby categorical values were reported using frequencies and percentages. McNemar’s test was used to compare changes in knowledge across the multiple administrations of the breast cancer-related knowledge surveys. A p-value less than 0.05 was considered significant for all the analysis.

Education and training of the HCWs

A total of 8 HCWs were trained. Tables  2 and 3 show the changes in breast cancer-related knowledge after the educational and training intervention for the HCWs. The absolute percentage increase in HCWs who correctly believed that breast cancer can occur in men, and in women despite breast feeding their children, was 50%. In addition, the percentage of respondents who believed that women with a painless lump should visit a healthcare professional increased from 87.5 to 100%. The absolute percentage of HCWs who correctly identified painless lump and bloody nipple discharge as a symptom suspicious of breast cancer increased by 12.5% and those that identified dimpling of skin as a suspicious symptom increased by 25%. The percentage of HCWs who correctly believed that a tissue biopsy could be used to diagnose breast cancer increased from 62.5 to 87.5%.

Implementation of the outreach program

A total of 8,757 women were screened by the HCWs in the field during initial series of home visits. A palpable breast lump was identified in 20/8,757 women, while other palpable or visible breast concerns warranting further evaluation were identified in 98/8,757 women. In addition, HCWs were unsure about the presence of a lump in 51/8,757 women. Keeping a low threshold for seeking a physician’s evaluation and prompt referrals, these 169/8,757 patients were all referred to a gynecologist for further examination. However, only 38/169 patients (ten with a palpable breast lump and 28 for which the HCWs exercised caution-either noted other breast concerns or were unsure) complied with initial referral to the gynecologist. Out of the 28 patients (where HCWs noted breast concerns or were unsure about a lump), none were found to have a lump on the gynecologist’s CBE examination. Out of the ten patients in which the HCWs had positively identified the breast lumps, nine patients (90% concordance) were confirmed to have a breast lump on the gynecologist’s CBE. However, all these ten patients were referred for imaging with only 4 of them complying. Amongst these 4 patients who had breast imaging, one patient had BI-RADS (Breast Imaging Reporting and Data System) category I finding (i.e. negative imaging) and 3 patients had BI-RADS category III findings (Lump with extremely low probability of malignancy). The outcomes of the CBE and referral program are illustrated in Fig.  1 .

At the follow-up home visits to the 131 patients who had been non-compliant with initial referral, the most common reasons for non-compliance were assessed by the HCWs ( Table  4 ). The most common reasons for non-compliance were a belief that follow-up was not important (42.0%), lack of money to visit the gynecologist (24.4%), not having anyone to accompany them (9.2%), long distance to travel for the appointment (7.6%).

Increase in community awareness regarding breast cancer

A comparison of the women’s knowledge regarding breast cancer at the time of initial visit and later at follow-up is shown in Table  5 . The percentage of women who had heard of breast cancer increased from 54.6 to 100%, the percentage of women who were aware that breast cancer was treatable increased from 32.8 to 61.3%. The percentage of women understood the need to consult a healthcare professional upon finding a lump increased from 50.4 to 94.1%.

The purpose of this study was to evaluate the feasibility of the real-world implementation of a large-scale clinical breast examination and referral community outreach program in a rural district of Pakistan. Secondarily, we also explored the feasibility of delivering basic breast cancer-related knowledge to the community via non-physician HCWs. This program was the first of its kind for breast cancer detection in the country. The key positive takeaways from our experience were that it is: (i) possible to train non-physician HCWs to perform a comprehensive CBE and identify examination findings warranting referral and further evaluation, (ii) practically feasible to implement a large-scale community outreach program with home-visits for mass detection of breast cancer, (iii) possible to increase community knowledge and awareness for breast cancer by imparting education at the home-visits when CBE was performed. However, we encountered several real-world challenges that precluded the full realization of this outreach program’s impact. Only 50% of women initially identified by the HCWs as having a breast lump followed through with referral to the gynecologist, and only 40% of women followed up with subsequent referral for imaging. None of the patients eventually referred for histopathological evaluation ended up complying with the referral. However, prior experience with a similar program by the AKDN in Tajikistan demonstrated that with appropriate follow-ups, breast cancer may be detected in up to 0.2% of the women in the community [ 29 ]. Although this rate is slightly lower than the reported incidences of mammographic screening-detected breast cancers in the literature (0.5–0.8%), it underscores the potential for success of CBE-based programs as an early detection strategy in low-resource communities [ 30 , 31 ].

Accuracy of CBE by non-physician HCWs and effectiveness of educational interventions

Overall, the theoretical frameworks and foundations of this large-scale clinical breast examination and referral community outreach program were observed to be largely successful. We were able to achieve a high degree of concordance (90%) between the CBE findings of the HCWs and the gynecologist, indicating that it is possible and feasible to leverage HCWs for the early detection of symptomatic breast cancer. Another study carried out in Malawi to train community laywomen to conduct CBE in the community showed 88% concordance between CBE performed by the HCWs and those performed by the physicians [ 32 ]. This is exceedingly important in a LMIC like Pakistan, where the ratio of physicians to population is a major impediment to healthcare access. In Pakistan, there are only 170,000 general practitioners to serve a population of over 230 million individuals. Thus, a major bottleneck for the delivery of high quality breast cancer-related healthcare is the timely initial identification of these patients from the community. Utilizing existing community outreach frameworks, such as the Lady Health Worker (LHW) Program, cite which was in Pakistan in 1994 [ 33 , 34 ]. While the LHW Program was initially developed for promoting family planning and maternal health, the model has been adapted for other major public health interventions such as immunizations and basic preventative healthcare. These LHWs are salaried and recognized as part of the healthcare workforce. Since LHWs are recruited from within the community itself, one of the major strengths of such a program is their ability to deliver culturally appropriate healthcare to populations with limited access to healthcare facilities. Thus, based on the successful training of HCWs in our study, we believe that the LHW Program model can be effectively adapted for the early recognition of breast abnormalities in women who would otherwise go undetected. However, it is important to know that the training of the HCWs in our study was performed by a fellowship-trained breast surgeon at a tertiary care hospital in one of the major cities of Pakistan. To ensure the feasibility, uptake, and growth of our model throughout the underserved regions of the country, it is important that a certain degree of sustainability is achieved. In future iterations of this model, we plan to assess the effectiveness of cascade learning with peer-to-peer teaching. In such a model, HCWs initially trained by a breast surgeon will subsequently assume the role of trainers themselves and teach other HCWs/LHWs how to perform a CBE. Interestingly, the study conducted in Malawi trained non-HCWs to serve as “Breast Health Workers”, highlighting the potential to leverage non-HCW professionals to perform a health-related role in communities with low HCW-to-patient ratios [ 32 ].

Community education and awareness

Despite the successful and rigorous implementation of the CBE and referral community outreach program, the Achilles’ heel of this project was the pervasive lack of community awareness regarding the importance of following up with referrals. This was compounded by other sociocultural barriers such as financial constraints, transportation issues, and a lack of family support to visit the healthcare facility. Thus, it is important that future iterations of similar public health interventions be cognizant of these challenges and seek to mitigate them to the best of their ability. Indeed, the most modifiable of these obstacles is the lack of awareness which can be countered by greater community education during home visits, with a particular focus on emphasizing the potential consequences of non-compliance with diagnostic evaluations. Our results demonstrated the feasibility of educating HCWs to subsequently serve as teachers for the community, and that the newly gained knowledge remained reasonably intact even at a follow-up of six months. A study in Vietnam showed that repeated breast cancer-related educational interventions were successful in increasing compliance with referrals for breast cancer evaluation. [ 35 ] In addition, a more robust follow-up system including frequent interaction and monitoring of patients could help boost compliance with referrals and better continuity of care. For example, routinely scheduled phone calls could be made to the patient as a reminder to follow-up with their referrals. Moreover, for women who are not able to comply with their referrals because of the absence of a family member to accompany them, arrangements may be made whereby LHWs could accompany them as their attendants.

The other challenges, however, harken to well-known and longstanding problems with the healthcare system in Pakistan, where most of the population is unable to afford even basic healthcare. In such a setting, Universal Healthcare Coverage (UHC) emerges as the only viable solution to the masses. An attempt at such a system, the Sehat Sahulat Program (SSP; translates to Health Facility Program) was introduced in 2016 by the provincial government of Khyber Pakhtunkhwa, one of the five provinces of Pakistan [ 36 ]. The SSP was designed to cover a broad range of health conditions and services, including breast cancer diagnosis and evaluation. While the program was met with success in its initial years, and even expanded into some of the other provinces, instability in the political and economic infrastructures of Pakistan have limited its growth, uptake, and effectiveness. Ideally, mass community interventions for early breast cancer detection such as ours could be integrated with UHC programs such as SSP to ensure patient compliance, continuity of care, and maximization of invested resources most effectively.

Limitations

Our study has several limitations that we would like to acknowledge. Firstly, we were unable to calculate a study participation rate as the HCWs did not record the number of informed refusals that they received from women in the community. Secondly, as mentioned earlier, compliance with referrals was exceedingly poor and limited the realization of the true impact of the program. Thirdly, given the limited number of HCWs included, we were unable to perform statistical comparisons to evaluate the improvement in HCWs knowledge. Lastly, the evaluation of the long-term impact and sustainability of the program was limited, presumably due to the influence of sociocultural barriers on the health-seeking behaviors of the women.

This study describes the real-world implementation of a large-scale clinical breast examination and referral community outreach program in a rural district of Pakistan. Our study highlights the importance of CBE programs in early recognition of breast abnormalities/lumps, in regions where mammography is not feasible. Such training programs may lay the foundation for improved provider and community awareness, and examination at the patient’s doorstep and initiate referrals. However, for such programs to ultimately lead to earlier detection of breast cancer/downstaging of disease, community awareness and political buy-in from governmental stakeholders would be critical. Lastly, for such a program to have a truly national impact and be sustainable, more widespread training of HCWs using cascade learning and peer-to-peer teaching models would be necessary.

Nine companies have been selected to conduct early-stage studies of concepts for commercial services to support lower-cost, higher-frequency missions to the Red Planet.

NASA has identified nine U.S. companies to perform a total of 12 concept studies of how commercial services can be applied to enable science missions to Mars. Each awardee will receive between $200,000 and $300,000 to produce a detailed report on potential services — including payload delivery, communications relay, surface imaging, and payload hosting — that could support future missions to the Red Planet.

The companies were selected from among those that responded to a Jan. 29 request for proposals from U.S. industry.

NASA’s Mars Exploration Program initiated the request for proposals to help establish a new paradigm for missions to Mars with the potential to advance high-priority science objectives. Many of the selected proposals center on adapting existing projects currently focused on the Moon and Earth to Mars-based applications.

They include “space tugs” to carry other spacecraft to Mars, spacecraft to host science instruments and cameras, and telecommunications relays. The concepts being sought are intended to support a broad strategy of partnerships between government, industry, and international partners to enable frequent, lower-cost missions to Mars over the next 20 years.

“We’re in an exciting new era of space exploration, with rapid growth of commercial interest and capabilities,” said Eric Ianson, director of NASA’s Mars Exploration Program. “Now is the right time for NASA to begin looking at how public-private partnerships could support science at Mars in the coming decades.”

The selected Mars Exploration Commercial Services studies are divided into four categories:

Small payload delivery and hosting services

  • Lockheed Martin Corporation, Littleton, Colorado — adapt a lunar-exploration spacecraft
  • Impulse Space, Inc., Redondo Beach, California — adapt an Earth-vicinity orbital transfer vehicle (space tug)
  • Firefly Aerospace, Cedar Park, Texas — adapt a lunar-exploration spacecraft

Large payload delivery and hosting services

  • United Launch Services (ULA), LLC, Centennial, Colorado — modify an Earth-vicinity cryogenic upper stage
  • Blue Origin, LLC, Kent, Washington — adapt an Earth- and lunar-vicinity spacecraft
  • Astrobotic Technology, Inc., Pittsburgh — modify a lunar-exploration spacecraft

Mars surface-imaging services

  • Albedo Space Corporation, Broomfield, Colorado — adapt a low Earth orbit imaging satellite
  • Redwire Space, Inc., Littleton, Colorado — modify a low Earth orbit commercial imaging spacecraft
  • Astrobotic Technology, Inc. — modify a lunar exploration spacecraft to include imaging

Next-generation relay services

  • Space Exploration Technologies Corporation (SpaceX), Hawthorne, California — adapt Earth-orbit communication satellites for Mars
  • Lockheed Martin Corporation — provide communication relay services via a modified Mars orbiter
  • Blue Origin, LLC — provide communication relay services via an adapted Earth- and lunar-vicinity spacecraft

The 12-week studies are planned to conclude in August, and a study summary will be released later in the year. These studies could potentially lead to future requests for proposals but do not constitute a NASA commitment.

NASA is concurrently requesting separate industry proposals for its Mars Sample Return campaign, which seeks to bring samples being collected by the agency’s Perseverance rover to Earth, where they can be studied by laboratory equipment too large and complex to bring to Mars. The MSR industry studies are completely independent of the MEP commercial studies.

NASA’s Jet Propulsion Laboratory in Southern California manages the Mars Exploration Program on behalf of NASA’s Science Mission Directorate in Washington. The goal of the program is to provide a continuous flow of scientific information and discovery through a carefully selected series of robotic orbiters, landers, and mobile laboratories interconnected by a high-bandwidth Mars-Earth communications network. Scientific data and associated information for all Mars Exploration Program missions are archived in the NASA Planetary Data System.

WASHINGTON (AP) — The U.S. Drug Enforcement Administration is moving toward reclassifying marijuana as a less dangerous drug. The Justice Department proposal would recognize the medical uses of cannabis , but wouldn’t legalize it for recreational use.

The proposal would move marijuana from the “Schedule I” group to the less tightly regulated “Schedule III.”

So what does that mean, and what are the implications?

WHAT HAS ACTUALLY CHANGED? WHAT HAPPENS NEXT?

Technically, nothing yet. The proposal must be reviewed by the White House Office of Management and Budget, and then undergo a public-comment period and review from an administrative judge, a potentially lengthy process.

Still, the switch is considered “paradigm-shifting, and it’s very exciting,” Vince Sliwoski, a Portland, Oregon-based cannabis and psychedelics attorney who runs well-known legal blogs on those topics, told The Associated Press when the federal Health and Human Services Department recommended the change.

“I can’t emphasize enough how big of news it is,” he said.

It came after President Joe Biden asked both HHS and the attorney general, who oversees the DEA, last year to review how marijuana was classified. Schedule I put it on par, legally, with heroin, LSD, quaaludes and ecstasy, among others.

Biden, a Democrat, supports legalizing medical marijuana for use “where appropriate, consistent with medical and scientific evidence,” White House press secretary Karine Jean-Pierre said Thursday. “That is why it is important for this independent review to go through.”

IF MARIJUANA GETS RECLASSIFIED, WOULD IT LEGALIZE RECREATIONAL CANNABIS NATIONWIDE?

AP correspondent Haya Panjwani reports on a proposal for the federal government to reclassify marijuana in what would be a historic shift that could have wide ripple effects across the country.

No. Schedule III drugs — which include ketamine, anabolic steroids and some acetaminophen-codeine combinations — are still controlled substances.

They’re subject to various rules that allow for some medical uses, and for federal criminal prosecution of anyone who traffics in the drugs without permission.

No changes are expected to the medical marijuana programs now licensed in 38 states or the legal recreational cannabis markets in 23 states, but it’s unlikely they would meet the federal production, record-keeping, prescribing and other requirements for Schedule III drugs.

There haven’t been many federal prosecutions for simply possessing marijuana in recent years, even under marijuana’s current Schedule I status, but the reclassification wouldn’t have an immediate impact on people already in the criminal justice system.

“Put simple, this move from Schedule I to Schedule III is not getting people out of jail,” said David Culver, senior vice president of public affairs at the U.S. Cannabis Council.

But rescheduling in itself would have some impact, particularly on research and marijuana business taxes.

WHAT WOULD THIS MEAN FOR RESEARCH?

Because marijuana is on Schedule I, it’s been very difficult to conduct authorized clinical studies that involve administering the drug. That has created something of a Catch-22: calls for more research, but barriers to doing it. (Scientists sometimes rely instead on people’s own reports of their marijuana use.)

Schedule III drugs are easier to study, though the reclassification wouldn’t immediately reverse all barriers to study.

“It’s going to be really confusing for a long time,” said Ziva Cooper, director of the University of California, Los Angeles Center for Cannabis and Cannabinoids. “When the dust has settled, I don’t know how many years from now, research will be easier.”

Among the unknowns: whether researchers will be able to study marijuana from state-licensed dispensaries and how the federal Food and Drug Administration might oversee that.

Some researchers are optimistic.

“Reducing the schedule to schedule 3 will open up the door for us to be able to conduct research with human subjects with cannabis,” said Susan Ferguson, director of University of Washington’s Addictions, Drug & Alcohol Institute in Seattle.

WHAT ABOUT TAXES (AND BANKING)?

Under the federal tax code, businesses involved in “trafficking” in marijuana or any other Schedule I or II drug can’t deduct rent, payroll or various other expenses that other businesses can write off. (Yes, at least some cannabis businesses, particularly state-licensed ones, do pay taxes to the federal government, despite its prohibition on marijuana.) Industry groups say the tax rate often ends up at 70% or more.

The deduction rule doesn’t apply to Schedule III drugs, so the proposed change would cut cannabis companies’ taxes substantially.

They say it would treat them like other industries and help them compete against illegal competitors that are frustrating licensees and officials in places such as New York .

“You’re going to make these state-legal programs stronger,” says Adam Goers, of The Cannabist Company, formerly Columbia Care. He co-chairs a coalition of corporate and other players that’s pushing for rescheduling.

It could also mean more cannabis promotion and advertising if those costs could be deducted, according to Beau Kilmer, co-director of the RAND Drug Policy Center.

Rescheduling wouldn’t directly affect another marijuana business problem: difficulty accessing banks, particularly for loans, because the federally regulated institutions are wary of the drug’s legal status. The industry has been looking instead to a measure called the SAFE Banking Act . It has repeatedly passed the House but stalled in the Senate.

ARE THERE CRITICS? WHAT DO THEY SAY?

Indeed, there are, including the national anti-legalization group Smart Approaches to Marijuana. President Kevin Sabet, a former Obama administration drug policy official, said the HHS recommendation “flies in the face of science, reeks of politics” and gives a regrettable nod to an industry “desperately looking for legitimacy.”

Some legalization advocates say rescheduling weed is too incremental. They want to keep the focus on removing it completely from the controlled substances list, which doesn’t include such items as alcohol or tobacco (they’re regulated, but that’s not the same).

Paul Armentano, the deputy director of the National Organization for the Reform of Marijuana Laws, said that simply reclassifying marijuana would be “perpetuating the existing divide between state and federal marijuana policies.” Kaliko Castille, a past president of the Minority Cannabis Business Association, said rescheduling just “re-brands prohibition,” rather than giving an all-clear to state licensees and putting a definitive close to decades of arrests that disproportionately pulled in people of color.

“Schedule III is going to leave it in this kind of amorphous, mucky middle where people are not going to understand the danger of it still being federally illegal,” he said.

Title: better & faster large language models via multi-token prediction.

Abstract: Large language models such as GPT and Llama are trained with a next-token prediction loss. In this work, we suggest that training language models to predict multiple future tokens at once results in higher sample efficiency. More specifically, at each position in the training corpus, we ask the model to predict the following n tokens using n independent output heads, operating on top of a shared model trunk. Considering multi-token prediction as an auxiliary training task, we measure improved downstream capabilities with no overhead in training time for both code and natural language models. The method is increasingly useful for larger model sizes, and keeps its appeal when training for multiple epochs. Gains are especially pronounced on generative benchmarks like coding, where our models consistently outperform strong baselines by several percentage points. Our 13B parameter models solves 12 % more problems on HumanEval and 17 % more on MBPP than comparable next-token models. Experiments on small algorithmic tasks demonstrate that multi-token prediction is favorable for the development of induction heads and algorithmic reasoning capabilities. As an additional benefit, models trained with 4-token prediction are up to 3 times faster at inference, even with large batch sizes.

The agency tasked with regulating aviation and commercial space travel intends to begin an environmental impact study that could pave the way for SpaceX to begin Starship operations at the Kennedy Space Center.

The space company’s proposal includes conducting Starship-Super Heavy launches and performing recoverable booster landings along Florida’s east coast.

A previous environmental assessment was completed by NASA in 2019, but the FAA notice says the scope of the project has evolved since the original proposals.

"NASA is seeking the support of the Starship-Super Heavy at KSC in its continued mission to expand commercial uses of space and the space industry by facilitating SpaceX efforts to strengthen U.S. space transportation and launch infrastructure and providing greater mission capability to NASA and SpaceX by continuing the development of ever evolving next generation launch vehicles and spacecraft," the notice stated.

The EIS will analyze potential impacts to air quality, wildlife, water resources and noise pollution along the Florida Space Coast.

SpaceX envisions its program will one day enable cost-effective crewed missions to the Moon and Mars.

The company has launched the nearly 400-foot-tall Starship rocket three times from its facility in Boca Chica, Texas.

Each attempt has ended in a mishap that has triggered FAA investigations and caused scientists to conduct modifications for each subsequent mission.

SpaceX had planned to launch its fourth test flight in May with the goal of successfully recovering the booster and spacecraft.

A launch of a Starship with Starlink satellites is widely anticipated to happen this year – a task that has been performed by the company’s Falcon rocket series.

According to the newest proposal, the Kennedy Space Center could see up to 44 launches per year, up from the originally planned 24 launches.

Neither the FAA nor Elon Musk has announced an intended date for the Florida facility to be up and running.

Both the South Texas facility and its future location in Central Florida provide rockets with a flight path over water to minimize potential impacts and are far enough south to take advantage of Earth’s rotational speed.
