In my family, my father is a doctor and helps many people in a day. I also like to help people. It's disturbed me and it also encourages me to become a doctor so that I can help and provide services to people.

Being a doctor has always been my dream. I have always been fascinated by the human body and how it works, and the idea of being able to help people overcome their illnesses and lead healthier lives is what drives me to pursue a career in medicine.

There were so many people who died during COVID because of not getting treatment. There was a lack of doctors and nurses in the hospitals. I would have isolated myself from my family and treated the patients by staying at the hospital only. I would have spread awareness among the people regarding the cause of the spread of coronavirus and told them how they could save themselves.

If I were a doctor, I would recommend people opt for a healthy lifestyle. I would motivate them to do daily yoga and exercise for at least 30 minutes. People should devote themselves to physical exercise if they want to stay healthy and fit. They should eat healthy food and follow a properly balanced diet. I would recommend people cut down on fast food and street food. I would organise awareness campaigns related to the consumption of alcohol, tobacco, cigarettes, drugs etc. and educate people regarding their harmful effects.

Doctors are considered gods as they bring back life and treat illness. A good doctor is a blessing to society and the country, and that is why I would love to be a doctor.

That is a question I came to hate as I grew up and started school. All my parent’s friends, any time we would or they would visit, would ask me that question. As a young child, I would make something up and say stupid things like I want to be a professional football player.

Or I would say that I wanted to be a circus performer . I got a kick out of their reactions when they heard those answers. The responses may not have been serious but eventually, it got my parents’ friends to quit asking me what I wanted to be when I grew up.

Then I hit my teen years and I started to see the serious nature behind that question and that realization got me thinking. What was I going to do with my life? I do not care for sports that much so being a professional athlete was out and I hate the circus so that career was off my list.

As I thought I explored my strengths and saw that I genuinely cared about people and I could stand the sight of blood. Then it hit me, I could be a doctor. My grades have always been good and people told me that I was very smart and both qualities were needed to become a doctor.

So I set my sights on being a doctor for the rest of my life. As I study I keep that one goal in mind. It is hard work but one day I will be able to enter medical school and learn how to be a doctor.

What I like about being a medical professional is not the money that is involved. The big money does not come to many doctors until they have become quite specialized and famous. There are lots of doctors that make very little money and seem quite content.

What really appeals to me is the positive contribution doctors make in society. They have the knowledge and talent to make a real difference in someone’s life. Plus, they can go to just about any country and practice their craft.

There always seems to be a need for doctors no matter where you go. That is a very attractive feeling to have. Being needed makes a person feel really good about themselves. Especially since there are so many specialties one can choose to work in.

Don’t get me wrong, I have not looked at this profession with rose-colored glasses on. I know that there is a negative side that takes its toll on doctors. Patients do not always get well, their family members get angry and treatments do not always go as planned and doctors get sued.

Those negatives aspects just motivate me to be the best doctor I can be and learn everything I can so I can avoid such situations if possible. My aim to be a doctor is quite legitimate and strong.

That is because I also see that profession as very rewarding. Not financially, although that does help some, but in the aspect that I can make a difference no matter where I practice medicine. If I focus and concentrate on helping my patients and they get better, then I have done my job.

The look on my patients’ faces when I give them the good news is more than enough inspiration and reward. Words cannot express such a feeling when you have helped someone and they are genuinely grateful.

All of these elements make my aim to be a doctor a legitimate goal and helps keep me honest. An honest doctor is the best one to have to look after you as you never get lied to and people can have confidence in him.

That is another reason for my aim to be a doctor. I like the idea that people can have confidence in me.

A survey shows that doctors have trouble taking full vacations from their high-stress jobs. Even when they do, they often still do work on their time off.

A few weeks ago, I took a vacation with my family. We went hiking in the national parks of southern Utah, and I was blissfully disconnected from work.

I'm a family physician, so taking a break from my job meant not seeing patients. It also meant not responding to patients' messages or checking my work email. For a full week, I was free.

Taking a real break — with no sneaky computer time to bang out a few prescription refill requests — left me feeling reenergized and ready to take care of my patients when I returned.

But apparently, being a doctor who doesn't work on vacation puts me squarely in the minority of U.S. physicians.

Research published in JAMA Network Open this year set out to quantify exactly how doctors use their vacation time — and what the implications might be for a health care workforce plagued by burnout, dissatisfaction and doctors who are thinking about leaving medicine.

"There is a strong business case for supporting taking real vacation," says Dr. Christine Sinsky , the lead author of the paper. "Burnout is incredibly expensive for organizations."

Researchers surveyed 3,024 doctors, part of an American Medical Association cohort designed to represent the American physician workforce. They found that 59.6% of American physicians took 15 days of vacation or less per year. That's a little more than the average American: Most workers who have been at a job for a year or more get between 10 and 14 days of paid vacation time , according to the U.S. Bureau of Labor Statistics.

However, most doctors don't take real vacation. Over 70% of doctors surveyed said they worked on a typical vacation day.

"I have heard physicians refer to PTO as 'pretend time off,'" Sinsky says, referring to the acronym for "paid time off."

Sinsky and co-authors found that physicians who took more than three weeks of vacation a year had lower rates of burnout than those who took less, since vacation time is linked to well-being and job satisfaction .

And all those doctors toiling away on vacation, sitting poolside with their laptops? Sinsky argues it has serious consequences for health care.

Physician burnout is linked to high job turnover and excess health care costs , among other problems.

Still, it can be hard to change the culture of workaholism in medicine. Even the study authors confessed that they, too, worked on vacation.

"I remember when one of our first well-being papers was published," says Dr. Colin West , a co-author of the new study and a health care workforce researcher at the Mayo Clinic. "I responded to the revisions up at the family cabin in northern Minnesota on vacation."

Sinsky agreed. "I do not take all my vacation, which I recognize as a delicious irony of the whole thing," she says.

She's the American Medical Association's vice president of professional satisfaction. If she can't take a real vacation, is there any hope for the rest of us?

I interviewed a half dozen fellow physicians and chatted off the record with many friends and colleagues to get a sense of why it feels so hard to give ourselves a break. Here, I offer a few theories about why doctors are so terrible at taking time off.

We don't want to make more work for our colleagues

The authors of the study in JAMA Network Open didn't explore exactly what type of work doctors did on vacation, but the physicians I spoke to had some ideas.

"If I am not doing anything, I will triage my email a little bit," says Jocelyn Fitzgerald , a urogynecologist at the University of Pittsburgh who was not involved in the study. "I also find that certain high-priority virtual meetings sometimes find their way into my vacations."

Even if doctors aren't scheduled to see patients, there's almost always plenty of work to be done: dealing with emergencies, medication refills, paperwork. For many of us, the electronic medical record (EMR) is an unrelenting taskmaster , delivering a near-constant flow of bureaucratic to-dos.

When I go on vacation, my fellow primary care doctors handle that work for me, and I do the same for them.

But it can sometimes feel like a lot to ask, especially when colleagues are doing that work on top of their normal workload.

"You end up putting people in kind of a sticky situation, asking for favors, and they [feel they] need to pay it back," says Jay-Sheree Allen , a family physician and fellow in preventive medicine at the Mayo Clinic.

She says her practice has a "doctor of the day" who covers all urgent calls and messages, which helps reduce some of the guilt she feels about taking time off.

Still, non-urgent tasks are left for her to complete when she gets back. She says she usually logs in to the EMR when she's on vacation so the tasks don't pile up upon her return. If she doesn't, Allen estimates there will be about eight hours of paperwork awaiting her after a week or so of vacation.

"My strategy, I absolutely do not recommend," Allen says. But "I would prefer that than coming back to the total storm."

We have too little flexibility about when we take vacation

Lawren Wooten , a resident physician in pediatrics at the University of California San Francisco, says she takes 100% of her vacation time. But there are a lot of stipulations about exactly how she uses it.

She has to take it in two-week blocks — "that's a long time at once," she says — and it's hard to change the schedule once her chief residents assign her dates.

"Sometimes I wish I had vacation in the middle of two really emotionally challenging rotations like an ICU rotation and an oncology rotation," she says, referring to the intensive care unit. "We don't really get to control our schedules at this point in our careers."

Once Wooten finishes residency and becomes an attending physician, it's likely she'll have more autonomy over her vacation time — but not necessarily all that much more.

"We generally have to know when our vacations are far in advance because patients schedule with us far in advance," says Fitzgerald, the gynecologist.

Taking vacation means giving up potential pay

Many physicians are paid based on the number of patients they see or procedures they complete. If they take time off work, they make less money.

"Vacation is money off your table," says West, the physician well-being researcher. "People have a hard time stepping off of the treadmill."

A 2022 research brief from the American Medical Association estimated that over 55% of U.S. physicians were paid at least in part based on "productivity," as opposed to earning a flat amount regardless of patient volume. That means the more patients doctors cram into their schedules, the more money they make. Going on vacation could decrease their take-home pay.

But West says it's important to weigh the financial benefits of skipping vacation against the risk of burnout from working too much.

Physician burnout is linked not only to excess health care costs but also to higher rates of medical errors. In one large survey of American surgeons , for example, surgeons experiencing burnout were more likely to report being involved in a major medical error. (It's unclear to what extent the burnout caused the errors or the errors caused the burnout, however.)

Doctors think they're the only one who can do their jobs

When I go on vacation, my colleagues see my patients for me. I work in a small office, so I know the other doctors well and I trust that my patients are in good hands when I'm away.

But ceding that control to colleagues might be difficult for some doctors, especially when it comes to challenging patients or big research projects.

"I think we need to learn to be better at trusting our colleagues," says Adi Shah , an infectious disease doctor at the Mayo Clinic. "You don't have to micromanage every slide on the PowerPoint — it's OK."

West, the well-being researcher, says health care is moving toward a team-based model and away from a culture where an individual doctor is responsible for everything. Still, he adds, it can be hard for some doctors to accept help.

"You can be a neurosurgeon, you're supposed to go on vacation tomorrow and you operate on a patient. And there are complications or risk of complications, and you're the one who has the relationship with that family," West says. "It is really, really hard for us to say ... 'You're in great hands with the rest of my team.'"

What doctors need, says West, is "a little bit less of the God complex."

We don't have any interests other than medicine

Shah, the infectious disease doctor, frequently posts tongue-in-cheek memes on X (formerly known as Twitter) about the culture of medicine. Unplugging during vacation is one of his favorite topics, despite his struggles to follow his own advice.

His recommendation to doctors is to get a hobby, so we can find something better to do than work all the time.

"Stop taking yourself too seriously," he says. Shah argues that medical training is so busy that many physicians neglect to develop any interests other than medicine. When fully trained doctors are finally finished with their education, he says, they're at a loss for what to do with their newfound freedom.

Since completing his training a few years ago, Shah has committed himself to new hobbies, such as salsa dancing. He has plans to go to a kite festival next year.

Shah has also prioritized making the long trip from Minnesota to see his family in India at least twice a year — a journey that requires significant time off work. He has a trip there planned this month.

"This is the first time in 11 years I'm making it to India in summer so that I can have a mango in May," the peak season for the fruit, Shah says.

Wooten, the pediatrician, agrees. She works hard to develop a full life outside her career.

"Throughout our secondary and medical education, I believe we've really been indoctrinated into putting institutions above ourselves," Wooten adds. "It takes work to overcome that."

The Simple Math That Could Swing the Election to Biden

Mr. Penn was a pollster and an adviser to President Bill Clinton and Hillary Clinton from 1995 to 2008. He is chairman of the Harris Poll and chief executive of Stagwell Inc.

President Biden appears behind in all the swing states, and his campaign appears all too focused on firming up his political base on the left with his new shift on Israel , a $7 trillion budget and massive tax increases and is failing to connect on the basic issues of inflation , immigration and energy . By pitching too much to the base, he is leaving behind the centrist voters who shift between parties from election to election and, I believe, will be the key factor deciding the 2024 race.

I’ve spent decades looking at the behavior of swing voters and how candidates appeal to them, including for Bill Clinton’s re-election campaign in 1996. If Mr. Biden wants to serve another four years, he has to stop being dragged to the left and chart a different course closer to the center that appeals to those voters who favor bipartisan compromises to our core issues, fiscal discipline and a strong America.

People usually assume that turning out so-called base voters in an election matters most, since swing voters are fewer in number. And it’s true that in today’s polarized environment, Mr. Biden and Mr. Trump have about 40 percent of voters each and nothing will change those people’s minds. But in that remaining 20 percent of the electorate, voters have disproportionate power because of their potential to switch. It’s simple math: Take a race tied in the run-up 5 to 5. If one voter swings, the tally becomes 6 to 4. Two voters would then need to be turned out just to tie it up, and a third one would be needed to win.

The simple power of this math — which drove the campaigns of Mr. Clinton (with his message about “building a bridge to the 21st century”), George W. Bush (“compassionate conservatism”) and Barack Obama (“hope and change”) — has been obscured, undoubtedly by base groups like unions or PACs that have a vested interest in maintaining their sway and power. Take Michigan, a battleground state where Mr. Trump has led Mr. Biden by as many as three percentage points in the past month. To overcome that gap, Mr. Biden would need to bring out nearly 250,000 additional voters (3 percent of more than eight million registered voters) just to tie it up in a state that has already achieved a record of over 70 percent turnout in a presidential year. Or Mr. Biden could switch just 125,000 swing voters and win.

Despite this math, scared candidates are, in my experience, easily sold the idea that the Democratic base or Republican base is going to stay home in November unless they are constantly fed what they want to hear. One call from the head of a religious group, a civil rights group, a labor group and others (often called the groups), and fear runs through a campaign. A New York Times article this winter about Black pastors warning the Biden White House that his Gaza war policy could imperil re-election is a good example. Maybe if Mr. Biden were running against a well-liked centrist opponent, concern could be justified. But during a fall election against Mr. Trump, the final month of this campaign is going to see a frenzy of get-out-the-vote efforts, and I doubt the Democratic base is going to sit idly by at the thought of the Trump limo cruising up Pennsylvania Avenue. The reality is that swing voters in battleground states who are upset about immigration, inflation and what they see as extreme climate policies and weakness in foreign affairs are likely to put Mr. Trump back in office if they are not blunted.

Consider some Democratic electoral history. Mr. Biden got 81 percent of the vote in the Michigan Democratic presidential primary in February. He got roughly similar percentages in the Colorado, Texas and Massachusetts primaries — not too far below other incumbent presidents with a weak job rating. And yet for months, liberal commentators and activists pointed to the Michigan protest vote as proof that he is doomed in November over his Israel stance. But Michigan was hardly a repeat of the 1968 New Hampshire primary that effectively ended Lyndon Johnson’s re-election bid; Eugene McCarthy got 42 percent, and that was a truly sizable protest.

I believe most of the 101,000 uncommitted votes that Mr. Biden lost in Michigan will come home in the end because they have nowhere else to go and the threat Mr. Trump poses will become clearer and scarier in the next six months. But regardless, there’s a much bigger opportunity for Mr. Biden if he looks in the other direction. Mr. Trump lost nearly 300,000 votes to Nikki Haley in the Michigan Republican primary. These people are in the moderate center, and many of them could be persuaded to vote for Mr. Biden if he fine-tuned his message to bring them in. And remember to multiply by two: Persuading those 300,000 Republicans to cross party lines would have the equivalent force of turning out 600,000 Democrats. The same math applies to other battleground states, like Pennsylvania, where 158,000 people voted for Ms. Haley instead of Mr. Trump in the Republican primary, even though she dropped out seven weeks earlier.

Unfortunately, Mr. Biden is not reaching out to moderate voters with policy ideas or a strong campaign message. He is not showing clear evidence of bringing in large numbers of swing voters in the battleground states at this point. Those swing voters look for fiscal restraint without tax increases, climate policies that still give people a choice of cars and fuels and immigration policies that are compassionate to those who are here but close the borders. The balanced budget remains one of the single strongest measures that swing and other voters want. Mr. Clinton’s efforts to balance the budget set off the revolution that resulted in an eight-point win, even with third-party candidates in 1996, and catapulted his job approval ratings to above 70 percent. Instead of pivoting to the center when talking to 32 million people tuned in to his State of the Union address, Mr. Biden doubled down on his base strategy with hits like class warfare attacks on the rich and big corporations, big tax increases, student loan giveaways and further expansions of social programs despite a deficit of more than $1.1 trillion. The results quickly dissipated.

Mr. Biden’s campaign has fundamentally miscalculated on Israel. Those Haley voters are strong defense voters who would back our ally Israel unreservedly and, I believe, want to see a president who would put maximum pressure on Hamas to release hostages. By pandering to base voters with no choice, Biden is pushing the Haley vote to Mr. Trump, and so his first instincts on Israel were both good policy and good politics. Eighty-four percent of independents surveyed said they supported Israel more than Hamas in the conflict, and 63 percent said they believed a cease-fire should occur only after the hostages have been released. The more Mr. Biden has pandered to the left by softening his support of Israel, the weaker he looks, and the more his foreign policy ratings have declined. Rather than pull decisively away from Israel, Mr. Biden should instead find a plan that enables Israel to go into Rafah and that has enough precautions for Rafah’s civilians so the American president can back it.

At this point, Mr. Biden also needs to give a serious speech on the issues of crime and immigration and what they are doing to our inner cities. He has to combine policies of fair policing and treatment of DACA recipients with tougher crime and immigration policies. Seventy-eight percent of independents polled said they wanted the Biden administration to make it tougher to get into the United States illegally, but 63 percent said they ultimately wanted compromise legislation that strengthens the border while giving DACA recipients a path to citizenship. On crime, despite many violent crime metrics returning to their pre-Covid levels last year, voters have been more worried than ever. Eighty-three percent of voters said they wanted shoplifting laws to be enforced strictly , and 69 percent said they supported Justice Department intervention against city district attorneys who are pulling back prosecution of violent offenders. Mr. Biden has to be more responsive to these concerns.

Mr. Biden’s energy policies, especially his push for more electric vehicles, are not popular, either. Fifty-nine percent of Americans said they opposed the mandate that half of cars sold in the United States by 2030 be electric. In Michigan, Mr. Trump has identified a potentially killer strategy by going around telling autoworkers that electric vehicles will destroy their jobs. Unlike foreign policy issues, threats concerning the loss of auto industry jobs could directly affect hundreds of thousands of voters in Michigan.

The 2024 election is a rematch, but Mr. Biden should not assume that he will get the same result as he did in 2020 in Michigan, Pennsylvania, Arizona, Georgia and other battleground states by running the same playbook. This time around, he is seen as older, and the assessment of the job that he has done is in negative territory. While he won’t get any younger, he could still move more to the center, vacuum up swing voters who desperately want to reject Mr. Trump, strengthen his image as a leader by destroying Hamas and rally the base at the end. But that means first pushing back against the base rather than pandering to it and remembering that when it comes to the math of elections, swing is king.
