Scope: The course is designed to train the students and improve professional skills to provide various pharmaceuticalcare services in community pharmacy.

Course Objectives: This course will train the students in the following

1. Professional handling and filling prescriptions

2. Patient counselling on diseases and minor ailments

3. Patient counselling on prescription and / or non-prescription medicines

4. Preparation of counselling materials such as patient information leaflets

5. Performing basic health screening tests

Course Outcomes: Upon successful completion of this course, the students will be able to

1. Handle and fill prescriptions in a professional manner

2. Counsel patients on various diseases and minor ailments

3. Counsel patients on prescription and or non-prescription medicines

4. Design and prepare patient information leaflets

5. Perform basic health screening tests

Note: The following practicals shall be carried out in the model community pharmacy with appropriate simulated scenarios and materials. Students shall be trained through role plays wherever necessary. The activities of the students shall be assessed / evaluated using a structured objective assessment form.

Handling of prescriptions with professional standards, reviewing prescriptions, checking for legal compliance and completeness (minimum 5)

Identification of drug-drug interactions in the prescription and follow-up actions (minimum 2)

Preparation of dispensing labels and auxiliary labels for the prescribed medications (minimum 5)

Providing the following health screening services for monitoring patients / detecting new patients (one experiment for each activity)

Blood Pressure Recording, Capillary Blood Glucose Monitoring, Lung function assessment using Peak Flow Meter and incentive spirometer, recording capillary oxygen level using Pulse Oximeter, BMI measurement

Providing counselling to simulated patients for the following chronic diseases / disorders including education on the use of devices such as insulin pen, inhalers, spacers, nebulizers, etc. where appropriate (one experiment for each disease)

Type 2 Diabetes Mellitus, Primary Hypertension, Asthma, Hyperlipidaemia, Rheumatoid Arthritis

Providing counselling to simulated patients for the following minor ailments (any three)

Headache, GI disturbances (Nausea, Vomiting, Dyspepsia, diarrhea, constipation), Worm infestations, Pyrexia, Upper Respiratory Tract infections, Skin infections, Oral and dental disorders.

Appropriate handling of dummy dosage forms with correct administration techniques - oral liquids with measuring cup/cap/dropper, Eye Drops, Inhalers, Nasal drops, Insulin pen, nebulizers, different types of tablets, patches, enemas, suppositories

Use of Community Pharmacy Software and digital health tools

Assignments

The students shall be asked to submit written assignments on the following topics (One assignment per student per sessional period. i.e., a minimum of THREE assignments per student)

SOPs for various activities in Community Pharmacy (as discussed in Theory and Practical)

List out the various abbreviations, short forms used in prescriptions and their interpretation

Patient Information Leaflet for a given chronic disease / disorder

Patient Information Leaflet for prescription / non-prescription medicines

Preparation of window / shelf display materials for the model community pharmacy

Overview of Software available for retail pharmacy management including billing, inventory, etc.

Dosage / Medication Reminder Aids

Overview on the operations and marketing strategies of various online pharmacies

Overview on the common fixed dose combinations

Overview on the medications requiring special storage conditions

These are the medication reminders and Smart Pill Dispensers that we think are worth the time to check into to see if they fit your situation.

Best Portable Reminder And Smart Pill Dispenser System

The E-Pill Pocket Pillbox is a vibrating pill timer reminder and dispenser.

The E-Pill Pocket Pillbox provides a vibration alert when it is time to take a medication. At the same time, it also gives an audible 30-second alarms to alert the senior.

You can tap the snooze feature to repeat the alarms up to 3 times, for a total of four audible alarms.

The E-Pill Pocket Pillbox has the capacity to hold up to 32 aspirin-sized tablets, but it is lightweight and small light enough to fit into your shirt pocket or a purse.

*TIP: If you are a senior who wears hearing aids or has a hard time hearing, E-Pill also has a lockable dispenser with a voice alert that says, “ Time to take your medications. ”

My father couldn’t hear certain tones, despite wearing hearing aids in both ears, so this would be a very cool feature for someone like him.

Best At Home Dispenser And Reminder System

MedMinder is a 7-day pillbox system for those who want a smart pill dispenser that offers reminders plus mobile alerts.

We like it because it is large – it has 28 compartments – so it can hold up to 4 daily doses of medication, which is enough for an entire week.

It even has its own cellular connection so that it doesn’t have to be connected to a phone line or internet.

The pros : What is cool about this system is that when it is time to take a medication, the MedMinder will flash a light in the appropriate compartment.

This makes it a great option for a hearing impaired senior.

If the person doesn’t remove the container within a certain time frame, the alert intensity really ratchets up.

For the second reminder, MedMinder flashes the light and also gives an audible tone, as well as sending the senior their preference of digital reminders (either a text message or an email) or initiating a phone call.

If you are a caretaker for the senior or are an adult child, you can program the system remotely.

In addition, it can be set up for remote monitoring and to alert a care provider if a dose is missed. MedMinder also generates monthly reports so caregivers can keep track of the senior’s medication activity.

We have been talking about the MedMinder unlocked version, but it also has a product with a safety lock feature so that children looking for candy or a senior with dementia won’t eat the medication by mistake.

The drawbacks : The system is not portable – it has to be plugged into an electrical outlet.

Best Medication Service

PillPack is an Amazon service that takes away the need for having to go to the pharmacy to pick up or refill a prescription.

This service sorts the senior’s monthly prescriptions (including over the counter supplements) and puts them into easy-open packets.

Each packet is clearly labeled with the day of the week and time of day the medication should be taken. P

illPack will also handle creams, inhalers, and testing supplies for the senior.

You can’t beat the price – the service and shipping are both free.

The senior only pays their regular insurance copays, just like they would normally have to do in a traditional pharmacy.

They also will need to pay out of pocket for any non-prescription medications (think Tylenol) or supplements and vitamins that they wish to include in the monthly service.

Pill Reminder Tops And Timer Caps For Prescription Bottles

Pill Reminder Tops can be set with up to 24 reminders per day (the reminders can only be set to the hour, though – not to a more specific time, like 10:24 a.m.

If this is a problem, you can get around it by changing the time on bottle – set it back (or forward) 15 or 30 minutes, then the alarms will go off at what they “think” is on the hour!).

The alarms repeat daily and the caps run on a LR 44 Button cell (included).

These tops record the last time the prescription bottle was opened and will fit on any medication bottle with a “standard 33 mm thread pharmaceutical closure”.

The tops come with a prescription bottle, though, just in case they do not fit on yours.

The Reminder Tops feature a flashing “missed dose” indicator, but a drawback is that they are not child proof.

Timer Caps have a stopwatch and a digital display that shows the last time the bottle was closed.

The stopwatch starts automatically when the top is put back on the bottle and continues to count up until the next time the pill bottle is opened, at which time it resets.

You don’t have to program them or set any alarms, but they can help seniors avoid taking a medication twice because of forgetting whether they took it the first time.

It also serves a secondary purpose of letting them know if anyone else has opened the medication (it is a theft deterrent) and will alert caregivers if someone tampered with the bottle.

Medication Management For Caregivers

In addition to the tools and services outlined above, there are a few other ways of providing medication management for caregivers:

• Day labeled pill boxes . That hold up to a one-week supply of medications. These are typically labeled with the days of the week and the period of the day (morning or breakfast, etc) in which the medication should be taken.
• Blister Packs or individually packaged doses: The pharmacy takes care of packaging one day’s worth of pills (or all the pills that need to be taken together at a certain time, such as at breakfast or dinner) into a blister pack. This way, the senior doesn’t have to open numerous bottles. It is similar to the Amazon PillPack service mentioned above, under the Best Medication Service section. Unlike with Amazon, the senior or caregiver will still have to visit the pharmacy to get their medications (Amazon’s service delivers it for free, right to the senior’s door).
• Alarmed pill boxes: These devices give an alarm to remind caregivers of when the senior is due to take their medications.

• Instructions on how to take (i.e. with or without food, serving size, method of ingestion, etc.)

Jan Aushadhi and other Generic Medicine initiatives in India

Over the years, India has developed a strong capability in producing quality branded and generic medicines in most of the therapeutic categories, evolving from a mere Rs 1,500 crores industry in 1980 to a more than Rs 1,19,000 crores industry in 2012. However, although these medicines are reasonably priced, as compared to the prices of their equivalent medicines in most other countries, yet a large population of poor people in the country find it difficult to afford the more expensive branded category of medicines.

Medicines in almost every therapeutic category are sold primarily as branded drugs, at disproportionately high prices. There is still a large section of the population which finds it difficult to afford these high-priced medicines. According to World Health Organization estimates (2008), 65% of India‟s population does not have access to modern health care. Since 80% of out-patient care and 60% of in-hospital care occurs at private hospitals in India, households are exposed to a private-sector market to buy drugs (Public Health Foundation of India, 2012). According to NSO estimates, upto 79% of health care expenses in rural areas are due to the cost of medicines.

This problem gets further aggravated as almost 80% of expenditure on health care is borne by the patients themselves. Thus, access to low-priced generic drugs is very critical in ensuring health care at affordable prices.

Introduction

With a view to achieve the objective of making available quality generic medicines at affordable prices to all, „Jan Aushadhi Scheme‟ was launched by the Department of Pharmaceuticals, Ministry of Chemicals & Fertilizers, Government of India in November, 2008 across the country. The Scheme is being implemented through the Bureau of Pharma PSUs of India (BPPI), under the administrative control of the Department of Pharmaceuticals, Ministry of Chemicals & Fertilizers, Government of India.

In September 2015, the ‘ Jan Aushadhi Scheme ‘ was revamped as “Pradhan Mantri Jan Aushadhi Yojana” (PMJAY). In November, 2016, to give further impetus to the scheme, it was again renamed as “ Pradhan Mantri Bhartiya Janaushadhi Pariyojana ” (PMBJP). Accordingly, ‘ensuring availability of quality medicines at affordable prices to all’, has been a key objective of the Government

The Salient features of the Jan Aushadhi Scheme are:

• Making quality medicines available at affordable prices for all, particularly for the the poor and disadvantaged, through specialized outlets called the Jan Aushadhi Stores (JAS).
• Provision of built up space for JAS in district hospitals by the State Governments.
• Operation of JAS by the State Government nominated Operating Agencies like NGOs, Charitable Organizations and public societies like the Red Cross Society and Rogi Kalyan Samitis, constituted for the purpose.
• Meeting the operational expenditure from the trade margins admissible for the medicines.
• To ensure prescription of generic medicines by the Government doctors with proactive support from the State Governments.

The Key objectives of Jan Aushadhi Scheme are:

• To make quality the hallmark of medicines by ensuring supplies from the Central Public Sector Undertaking (CPSUs) and also through other Public Sector Undertaking (PSUs) and Good manufacturing practices (GMP) compliant manufacturers in the private sector.
• Extend coverage of quality generic medicines, which would reduce and thereby redefine the unit cost of treatment per person.
• Provide access to any prescription drug or Over the Counter (OTC) drug in all therapeutic categories as generic equivalents, which is not to be restricted to the beneficiaries of Public Health System alone but also to serve others.
• Create awareness through education and publicity that quality is not synonymous with high price.

The Jan Aushadhi initiative makes available quality drugs at affordable prices through dedicated stores selling generic medicines which are available at lesser prices but are equivalent in quality and efficacy as expensive branded drugs. Some comparative advantages are:

• Promote greater awareness about cost-effective drugs and their prescription.
• Make available unbranded quality generic medicines at affordable prices through public private partnership.
• Encourage doctors, more specifically in government hospitals, to prescribe generic medicines.
• Enable substantial savings in health care, more particularly in the case of poor patients, and those suffering from chronic ailments, requiring long periods of drug use.

The Jan Aushadhi Campaig is a self sustaining business model not dependent on government subsidies or assistance. It is run in a Campaign Mission Mode on the principle of Not for Profits but with Minimal Profits. PMBJP seeks to keep product price within 50% of branded product price to make it affordable for all, especially the poor. It has resulted in substantial amount of savings for the people.

In the Budget Speech of 2016-17, opening 3,000 PMBJP Kendras was announced. As a result of the several actions and initiatives taken by the Government, 2060 PMBJP Kendras are functional in the 31 States/UTs of the country as on 27.07.2017 .

This has resulted in substantial savings to common man as prices of generic medicines being sold in the PMBJP Kendras is lower by 50% to 90% of the equivalent branded medicines.
